火针治疗寻常疣疗效的Meta分析及试验序贯分析

Meta Analysis and Trial Sequential Analysis of the Curative Effect of Fire Needle on Verruca Vulgaris

目的:系统评价火针治疗寻常疣的疗效和安全性。方法:计算机检索中国生物医学文献数据库(CBM)、中国知网(CNKI)、万方数据库、维普网(VIP)、Cochrane Library、PubMed、Springer link、EMbase、Web of Science等中英文数据库,收集关于火针治疗寻常疣的随机对照试验,检索时间范围设定为各数据库建库至2022年1月。根据Cochrane手册使用Ro B2评价纳入研究的方法学质量,运用RevMan5.4软件进行Meta软件分析,使用TSA0.9.5.10对结局指标进行试验序贯分析。结果:共纳入16篇文献2574例患者,其中试验组1533例,对照组1041例。Meta分析结果显示,火针治疗寻常疣的总有效率[RR=1.11,95%CI(1.04,1.19),P<0.01]和治愈率[RR=1.29,95%CI(1.12,1.48),P<0.01]优于对照组,差异有统计学意义。但在不良反应发生率、治愈后复发率和治疗时疼痛评分的差异与对照组相当,差异无统计学意义。试验序贯分析结果表明,火针治疗寻常疣总有效率和治愈率结果的证据确切。但在不良反应发生率、治愈后复发率和治疗时疼痛评分的TSA结果尚不明确,且样本量远低于所需的理想样本量。结论:火针治疗寻常疣的疗效确切,安全性高,值得推广使用,但远期疗效还需进一步验证。

Objective:To systematically evaluate the efficacy and safety of fire needle in the treatment of verruca vulgaris.Methods:China Biology Medicine disc(CBMdisc),China National Knowledge Infrastructure(CNKI),Wanfang Database,VIP,Cochrane Library,PubMed,Springer link,EMBASE,web of science and other Chinese and English databases were searched by computer to collect randomized controlled trials on fire acupuncture in the treatment of verruca vulgaris.The search time range was set from the establishment of each database to January 2022.According to Cochrane manual,ROB2 was used to evaluate the quality of methodology included in the study,revman5.4 software was used for meta software analysis,and TSA0.9.5.10 was used for sequential analysis of outcome indicators.Results:A total of 2574 patients in 16 literatures were included,including 1533 patients in the experimental group and 1041 patients in the control group.The results of Meta-analysis showed that the total effective rate [RR=1.11,95%CI(1.04,1.19),P<0.01] and cure rate [RR=1.29,95%CI(1.12,1.48),P<0.01] of fire needle in the treatment of verruca vulgaris were better than those in the control group,and the difference was statistically significant.However,there was no significant difference in the incidence of adverse reactions,recurrence rate after cure and pain score between the two groups,and the differences were not statistically significant.The results of sequential analysis showed that the evidence of the total effective rate and cure rate of fire needle in the treatment of verruca vulgaris was accurate.However,the TSA results of the adverse reaction rate,recurrence rate after cure and pain score during treatment are not clear,and the sample size is far lower than the required ideal sample size.Conclusion:Fire needle is effective and safe in the treatment of verruca vulgaris,which is worth popularizing,but the long-term curative effect needs to be further verified.
