脑内血肿清除去骨瓣手术与保守治疗高血压脑出血患者的效果对比

Comparison of the Effect of Intracerebral Hematoma Debridement and Conservative Treatment of Patients with Hypertensive Intracerebral Hem⁃orrhage

目的对比脑内血肿清除去骨瓣手术与保守治疗高血压脑出血患者效果。方法选取2017年8月—2021年8月于北京市丰台中西医结合医院治疗的74例高血压脑出血患者,按照治疗方式不同分为对照组、观察组,每组37例。对照组给予保守治疗,观察组给予脑内血肿清除去骨瓣手术治疗,比较两组患者的日常生活能力(ADL)评分、美国国立卫生院神经功能缺损评分(NIHSS)及治疗效果。结果观察组患者ADL评分(77.8±9.2)分显著高于对照组(59.4±8.7)分,差异有统计学意义(t=8.839,P<0.05);观察组患者NIHSS评分(11.75±2.34)分明显优于对照组(20.32±1.97)分,差异有统计学意义(t=17.042,P<0.05);观察组患者治疗优良率86.49%显著高于对照组62.16%,差异有统计学意义(χ^(2)=5.736,P<0.05)。结论与保守治疗相比,脑内血肿清除去骨瓣手术治疗高血压脑出血患者效果明显,能改善患者神经功能,有利于患者提高日常生活能力,改善患者生活质量。

Objective To study and compare the effect of intracerebral hematoma debridement with craniotomy and conservative treatment in patients with hypertensive cerebral hemorrhage.Methods A total of 74 patients with hyper⁃tensive intracerebral hemorrhage who were treated in Fengtai Hospital of Integrated Chinese and Western Medicine of Beijing City from August 2017 to August 2021 were selected and divided into control group and observation group,according to different treatment methods,with 37 cases in each group.The control group was given conservative treat⁃ment,and the observation group was given intracerebral hematoma removal and bone flap surgery treatment.The ability of daily living(ADL)score,national institutes of health stroke scale NIHSS and treatment effect of the two groups were compared.Results The ADL score of the observation group was(77.8±9.2)points significantly higher than that of the control group(59.4±8.7)points,the difference was statistically significant(t=8.839,P<0.05).The NIHSS score of the observation group(11.75±2.34)points was significantly better than that of the control group(20.32±1.97)points,the difference was statistically significant(t=17.042,P<0.05).The excellent and good rate of pa⁃tients in the observation group was 86.49%,which was significantly higher than that in the control group(62.16%),the difference was statistically significant(χ^(2)=5.736,P<0.05).Conclusion Compared with conservative treatment,in⁃tracerebral hematoma debridement and craniectomy in the treatment of hypertensive intracerebral hemorrhage pa⁃tients have obvious effect,can improve the neurological function of patients,help patients to improve their ability of daily living,and improve their quality of life.
