探析微创技术在口腔颌面血管畸形中临床疗效及安全性 被引量:2

Analysis of the Clinical Efficacy and Safety of Minimally Invasive Tech⁃niques in The treatment of Oral and Maxillofacial Vascular Malforma⁃tions

摘要 目的探析微创技术在口腔颌面血管畸形中临床疗效及安全性。方法选取临沂市中心医院2019年12月-2021年5月收治110例口腔颌面血管畸形患者,按照随机数表法分为对照组、观察组各55例,对照组在数字减影血管造影(digital subtract angiography,DSA)介导下行超选择性动脉内栓塞术(selective arterial emboli⁃zation,SAE)微创治疗,观察组在对照组治疗后1 d行彩色多普勒超声(colour doppler ultrasound examination,CDU)引导下平阳霉素(Pingyangmycin,PYM)注射微创治疗,高于对照组的对比两组临床疗效、不良反应发生情况、典型病例改善情况。结果观察组治疗有效率为96.36%、81.82%,组间比较差异有统计学意义(χ^(2)=5.986,P=0.014);观察组不良反应发生率为9.09%,对照组不良反应发生率为7.27%,组间比较差异无统计学意义(χ^(2)=0.000,P>0.05);与术前相比,观察组典型病例治疗后外形显著改善。结论DSA介导下行SAE微创治疗、CDU引导下PYM注射微创治疗等微创技术均在口腔颌面血管畸形中应用价值较高,对组织创伤较小,但SAE治疗后行PYM注射治疗能显著增强治疗效果,且不会叠加并发症,安全性、可行性较高。 Objective To explore the clinical efficacy and safety of minimally invasive techniques in the treatment of oral and maxillofacial vascular malformations.Methods A total of 110 patients with oral and maxillofacial vascular malformations were selected and treated in Linyi Central Hospital from December 2019 to May 2021.According to the random number table method,they were divided into control group and observation group with 55 cases in each group.The control group was treated with digital subtraction angiography(DSA)-mediated superselective arterial em⁃bolization(SAE)minimally invasive treatment,and the observation group received minimally invasive treatment with Pingyangmycin(PYM)injection under the guidance of color Doppler ultrasound(CDU)1 day after treatment in the control group.The clinical efficacy,occurrence of adverse reactions,and improvement of typical cases were com⁃pared between the two groups.Results The effective rates of the observation group was 96.36%higher than that of the control group 81.82%,and the difference between the groups was statistically significant(χ^(2)=5.986,P=0.014).The incidence of adverse reactions in the observation group was 9.09%,and the incidence of adverse reactions in the control group was 7.27%,and there was no significant difference between the groups(χ^(2)=0.000,P>0.05).Compared with before operation,the appearance of typical cases in observation group was significantly improved after treat⁃ment.Conclusion Minimally invasive techniques such as DSA-mediated SAE minimally invasive treatment and CDU-guided PYM injection minimally invasive treatment have high application value in oral and maxillofacial vas⁃cular malformations,with less tissue trauma,but PYM injection after SAE treatment can significantly improve En⁃hance the therapeutic effect without superimposing complications,with high safety and feasibility.
