复方血竭粉治疗糖尿病足溃疡创面的临床研究

Clinical Research of Fufang Xuejie Powder on the Treatment of Diabetic Foot Ulcer

目的:探讨复方血竭粉在促进治疗糖尿病足溃疡愈合中的作用。方法:选取2018年4月至2020年4月住院的糖尿病足患者,纳入符合标准的病例80例,其中治疗组及对照组各40例,两组病例基本资料及病情差异无统计学意义。治疗组采用清创换药+复合血竭粉方法,对照组采用单纯清创换药的方法。对比观察两组在临床综合疗效、患肢疼痛视觉模拟评分法(VAS)评分、红肿消退时间、创面愈合速率及脓液量等的差异。结果:临床综合疗效方面:治疗组治愈20例,显效6例,有效8例,无效6例,总有效率为85%;对照组治愈10例,显效9例,有效5例,无效16例,总有效率为60%;两组疗效比较差异有统计学意义(P<0.05)。两组在治疗后1个月及半年随访时VAS评分均较前明显改善,治疗组VAS评分由治疗前的(5.15±1.14)分降至治疗后1个月的(1.30±0.92)分及随访时的(0.65±0.75)分,对照组疼痛VAS评分由治疗前的(5.06±1.44)分降至治疗后1个月的(3.00±1.03)分及随访时的(1.69±0.95)分,两组之间差异有统计学意义(P<0.05)。治疗组在红肿消退时间及创面愈合时间方面明显优于对照组,差异有统计学意义(P<0.05),但两组脓液量差异无统计学意义(P>0.05)。结论:复方血竭粉在促进糖尿病足创口愈合、缓解创口疼痛、促进红肿消退等方面疗效显著,且优于一般清创换药,可治疗糖尿病足创面。

Objective:To explore the efficacy of Xuejie powder in promoting the healing of diabetic foot ulcers.Methods:Patients with diabetic foot patients hospitalized in hospital from April 2018 to April 2020 were selected including 80 eligible cases,with 40 cases in each group.There was no significant statistical difference in the basic data and the condition between two groups.The treatment group used the method of debridement and dressing Xuejie powder,and the control group used the method of debridement and dressing petrolatum ointment.The differences in clinical comprehensive efficacy,pain visual analogue scale(VAS)scores,redness,swelling subsided time,wound healing rate and pus volume were compared and observed between the two groups.Results:Clinical comprehensive efficacy:20 cases were cured in the treatment group,6 cases were markedly effective,8 cases were effective,6 cases were ineffective,and the total effective rate was 85%.In the control group,10 cases were cured,9 cases were markedly effective,5 cases were effective,16 cases were ineffective,and the total effective rate was 60%.The difference in efficacy between the two groups was significant(P<0.05).The VAS scores of the two groups were significantly improved at 1 month and half a year after treatment.The VAS scores of the treatment group decreased from 5.15±1.14 before treatment to 1.30±0.92 at 1 month after treatment and 0.65±0.75 at follow-up.The pain scores of the control group decreased from 5.06±1.44 before treatment to 3.00±1.03 at 1 month after treatment and 1.69±0.95 at follow-up,however,the difference between the two groups was statistically significant(P<0.05).The redness and swelling subsided time and wound healing time in treatment group were significantly better than that of the control group,and the difference was statistically significant(P<0.05),but there was no statistically significant difference in the amount of pus between the two groups(P>0.05).Conclusion:The Xuejie powder is effective in promoting the healing of diabetic foot wounds,relieving wound pain and promoting redness and swelling,which is better than the general debridement and dressing change group.It can treat diabetic foot ulcer.
