葶苈子的临床应用及其用量探究

通过搜集古代医籍及现代医家临证经验,总结出葶苈子具有以下特点:(1)临床汤剂多用3~45 g,常用剂量为10~30 g,丸剂用量多为0.375~5 g。(2)葶苈子泻肺平喘和利水消肿的功效相辅相成,不可完全分论,临床多用于治疗慢性阻塞性肺疾病、慢性支气管炎、哮喘、胸腔积液、水肿、慢性心衰等疾病,成人常用汤剂剂量为4.5~45 g,小儿常用汤剂剂量为3~10 g。(3)根据疾病、证型和症状,配伍相应中药,其功效有偏于气或偏于水者,如配伍麻黄、杏仁、桑白皮等药偏于降气,配伍牵牛子、防己、附子等药偏于利水。

By collecting the clinical experience of ancient medical books and modern physicians,it is concluded that pepperweed seed has the following characteristics:(1)Its clinical dosage range in decoctions is mostly 3-45 g,and its usual dosage range is 10~30 g,while in pills mostly 0.375-5 g.(2)The effects of pepperweed seed for purging the lung to relieve asthma and inducing diuresis for removing edema are complementary to each other and cannot be completely separated.Clinically,it is mostly used to treat chronic obstructive pulmonary disease,chronic bronchitis,asthma,pleural e?usion,edema,chronic heart failure and other diseases,with the usual clinical dosage range in decoctions for adults 4.5-45 g,while for children 3-10 g.(3)According to different diseases,syndrome types and symptoms,pepperweed seed is compatible with corresponding traditional Chinese medicines,differing in its bias toward descending qi or inducing diuresis.For example,it is combined with ephedra,bitter apricot seed,white mulberry root-bark,etc.,for mainly descending qi,while combined with pharbitis seed,mealy fangji root,prepared common monkshood branched root,etc.,for mainly inducing diuresis.
