针药结合治疗脑梗死后Ⅰ期肩手综合征

Exploration of the efficacy of acupuncture combined with medicines in the treatment of stageⅠshoulder-hand syndrome after cerebral infarction

目的探讨针药结合疗法治疗脑梗死后Ⅰ期肩手综合征疗效。方法选择脑梗死后Ⅰ期肩手综合征患者200例,按照随机数表法分为对照组与观察组,各100例。对照组予常规治疗,观察组在对照组基础上予中医联合治疗,比较2组治疗前后上肢运动功能、疼痛程度、生活能力评分、超声检查三项参数、血液流变学指标和肢体活动程度变化。结果治疗后,观察组FMA-U、VAS、MBI评分显著高于对照组(P<0.05);观察组血流速度、静脉内径改善程度显著优于对照组(P<0.05);观察组全血黏度高切、全血黏度低切、红细胞压积评分均显著低于对照组(P<0.05);观察组肩关节活动度显著优于对照组(P<0.05)。结论常规治疗方法联合针药结合疗法治疗脑梗死后Ⅰ期肩手综合征患者疗效明显。

Objective To explore the efficacy of acupuncture combined with medicines in the treatment of the stage I shoulder-hand syndrome after cerebral infarction.Methods A total of 200 patients with the stage I shoulder-hand syndrome after cerebral infarction were selected and randomly divided into a control group and an observation group,with 100 cases in each group.The control group was given a routine treatment,while the observation group was given a combined treatment of traditional Chinese medicine on the basis of that.The statistical differences of scores of upper limb motor function pain degree,life ability,the three parameters of ultrasound examination,hemorheology indexes and limb activity were compared between the two groups before and after treatment.Results After treatment,the scores of FMA-U,VAS and MBI in the observation group were significantly higher than those in the control group(P<0.05),and the improvement of blood flow velocity and vein diameter in the observation group were significantly better than that in the control group(P<0.05),while the scores of the whole blood viscosity high shear,whole blood viscosity low shear and hematocrit in the observation group were significantly lower than those in the control group(P<0.05),and the shoulder joint mobility in the observation group was significantly better than that in the control group(P<0.05).Conclusion On the basis of a conventional treatment,the combined treatment of TCM has an obvious curative effect in the treatment of patients with the stage I shoulder-hand syndrome after cerebral infarction.
