特应性皮炎伴发毛周角化症相关因素分析

Analysis of related factors of atopic dermatitis with keratosis pilaris

目的研究中国汉族特应性皮炎(AD)患者伴发毛周角化症(KP)的影响因素,为AD的诊治提供依据。方法自医院皮肤科及全国AD样本收集协作网收集的AD患者资料,应用统计学方法对各项数据进行分析。结果共收集了2956例AD患者,其中伴发KP者699例(23.6%)。分析显示,在性别、地域、慢性反复性皮炎、家族特应性疾病史、干皮症、掌纹症、眶周黑晕、鱼鳞病、出汗时瘙痒、白色划痕、疾病严重程度、发病类型方面,AD伴发KP与不伴发KP组间发病率有差异(P<0.05)。此外,地域、干皮症、掌纹症、眶周黑晕、鱼鳞病、白色划痕、皮损类型-肥厚型、疾病严重程度-中度,与AD伴发KP呈正相关(P<0.05)。其中地域(北方地区)、鱼鳞病、掌纹症、干皮症与AD伴发KP相关性较强。结论北方地区、掌纹症、鱼鳞病、干皮症是AD伴发KP的危险因素。

Objective To study the influencing factors of atopic dermatitis(AD)patients with keratosis pilaris(KP)in Chinese Han population,and to provide a basis for the diagnosis and treatment of AD.Methods The data of AD patients collected from the hospital dermatology department and the national AD sample collection network were analyzed by statistical methods.Results A total of 2956 AD patients were collected,of which 699(23.6%)were associated with KP.Analysis showed that in terms of gender,region,chronic repetitive dermatitis,family history of atopic diseases,xeroderma,palm prints,periorbital black halo,ichthyosis,itching during sweating,white scratches,disease severity,onset in terms of types,there was a difference in the incidence of AD with KP and without KP(P<0.05).In addition,geographic area,xeroderma,palm pattern,periorbital black halo,ichthyosis,white scratches,skin lesion type-hypertrophic type,disease severity-moderate were positively correlated with KP associated with AD(P<0.05).Among them,the region(northern region),ichthyosis,palm pattern,xeroderma were strongly correlated with AD associated with KP.Conclusion Northern areas,palm pattern,ichthyosis and xeroderma are risk factors for AD with KP.
