摘要
目的研究白细胞介素-33(IL-33)、金属蛋白酶组织抑制因子1(TIMP-1)及基质金属蛋白酶-2(MMP-2)联合乙肝五项指标诊断慢性HBV肝纤维化的应用价值。方法回顾性选取2020年1月至2021年12月于广州市第一人民医院南沙医院接受诊断的慢性HBV肝纤维化患者116例纳入研究组,肝纤维化分期:S1期16例,S2期39例,S3期47例,S4期14例。另同期体检健康志愿者116名纳入对照组。比较两组、研究组不同肝纤维化分期患者血清IL-33、TIMP-1、MMP-2水平。分析研究组乙肝五项患者在不同肝纤维化分期中的分布。分析乙肝五项、IL-33、TIMP-1、MMP-2与肝纤维化分期的相关性。结果研究组血清IL-33、TIMP-1、MMP-2水平为(562.86±59.30)pg/mL、(185.16±21.03)μg/L、(443.87±47.06)μg/L,均分别高于对照组[(142.96±17.35)pg/mL、(127.53±14.86)μg/L、(249.86±27.76)μg/L],差异均有统计学意义(P<0.05)。研究组不同肝纤维化分期血清IL-33、TIMP-1、MMP-2水平差异均有统计学意义(P<0.05)。研究组乙肝五项患者在不同肝纤维化分期患者中的分布差异均有统计学意义(P<0.05)。大三阳、小三阳及IL-33、TIMP-1、MMP-2与肝纤维化分期均呈正相关(r=0.430、0.368、0.462、0.538、0.528,P<0.05)。HBsAg阴性、1.4阳性、1.5阳性与肝纤维化分期均无相关性(P>0.05)。结论慢性HBV肝纤维化患者大三阳、小三阳及IL-33、TIMP-1、MMP-2与肝纤维化分期呈正相关,IL-33、TIMP-1、MMP-2联合乙肝五项指标对慢性HBV肝纤维化具有较高的诊断价值。
Objective To study the application value of interleukin-33(IL-33),tissue inhibitor of metalloproteinase 1(TIMP-1),matrix metalloproteinase(MMP-2)combined with hepatitis B in the diagnosis of chronic HBV liver fibrosis.Methods A total of 116 patients with chronic HBV liver fibrosis diagnosed in Nansha hospital,Guangzhou First People's Hospital from January 2020 to December 2021 were retrospectively included in the study group,liver fibrosis staging:16 cases in S1 stage,39 cases in S2 stage,47 cases in S3 stage,and 14 cases in S4 stage.And 116 healthy volunteers examined in our hospital during the same period were included in the control group.Serum IL-33,TIMP-1 and MMP-2 levels of patients with different LF stages were compared between the two groups and the study group.The distribution of five hepatitis B patients in different liver fibrosis stages in the study group was analyzed.The correlation between five hepatitis B items,IL-33,TIMP-1,MMP-2 and LF stage was analyzed.Results The levels of IL-33,TIMP-1 and MMP-2 in the study group were(562.86±59.30)pg/mL,(185.16±21.03)μg/L,(443.87±47.06)μg/L which were respectively higher than those in the control group[(142.96±17.35)pg/mL,(127.53±14.86)μg/L,(249.86±27.76)μg/L],the differences were statistically significant(P<0.05).The levels of IL-33,TIMP-1 and MMP-2 in different liver fibrosis stages of the study group were significantly different,the differences were statistically significant(P<0.05).There were significant differences in the distribution of five hepatitis B patients with different LF stages in the study group,the differences were statistically significant(P<0.05).There was a positive correlation between large and small three Yang,IL-33,TIMP-1,MMP-2 and LF stage(r=0.430,0.368,0.462,0.538,0.528;P<0.05).There was no correlation between HBsAg negative,1.4 positive,1.5 positive and liver fibrosis stage(P>0.05).Conclusion In patients with chronic HBV liver fibrosis,three positive,three negative positive and IL-33,TIMP-1,MMP-2 are positively correlated with THE stage of liver fibrosis,il-33,TIMP-1,MMP-2 combined with the five indicators of HEPATITIS B have high diagnostic value for chronic HBV liver fibrosis.
