水通道蛋白磁共振分子成像定量参数对阿尔茨海默病的诊断价值研究 被引量:1

Diagnostic values of aquaporin magnetic resonance molecular imaging quantitative parameters in Alzheimer's disease

摘要 目的探讨与研究水通道蛋白磁共振分子成像(AQP-MRI)定量参数对阿尔茨海默病的诊断价值。方法回顾性选择2020年10月至2021年10月在宁德师范学院附属宁德市医院进行诊治的轻度认知障碍患者35例与阿尔茨海默病患者24例分别作为B组与C组,同期选择在宁德师范学院附属宁德市医院进行体检的健康人15名作为A组。所有入选者均给予AQP-MRI检查,记录感兴趣区的水通道蛋白表观扩散系数(AQP-ADC)、ADC,判断诊断价值。结果A组MRI分级都正常,B组与C组的MRI分级对比差异有统计学意义(P<0.05)。C组与B组苍白球、黑质与丘脑AQP-MRI体素分别为51.33±5.39、8.22±0.43、35.32±5.18和61.04±9.67、10.56±0.89、44.79±6.92、均明显低于A组(78.09±5.02、17.83±4.21、62.98±5.77),C组明显低于B组,差异均有统计学意义(P<0.05)。C组与B组AQP-ADC、ADC分别为0.41±0.09、0.60±0.02和0.27±0.03、0.47±0.04,均明显高于A组(0.21±0.04、0.35±0.05),C组明显高于B组,差异均有统计学意义(P<0.05)。在C组24例入选者中,Spearman相关性分析显示,AQP-ADC、ADC与阿尔茨海默病呈正相关(r=0.612、0.672,P<0.05);ROC曲线分析显示,AQP-MRI AQP-ADC、ADC诊断阿尔茨海默病的曲线下面积分别为0.813、0.766。结论阿尔茨海默病患者多伴随有脑功能MRI分级改变,多伴随有AQP-MRI AQP-ADC、ADC升高,AQP-MRI定量参数诊断阿尔茨海默病具有很好的价值。 Objective To explore and study the diagnostic value of quantitative parameters of aquaporins-magnetic resonance imaging(AQP-MRI)in Alzheimer's disease(AD).Methods From October 2020 to October 2021,35 cases of patients with mild cognitive impairment and 24 cases of patients with Alzheimer's disease who were diagnosed and treated in Ningde Municipal Hospital Affiliated to Ningde Normal University were selected as group B and group C,and the other 15 cases of healthy people who underwent physical examination in Ningde Municipal Hospital Affiliated to Ningde Normal University during the same period were selected as group A.All the cases were given AQP-MRI examination,and the aquaporin apparent diffusion coefficient(AQP-ADC)and ADC of the region of interest were recorded to judge the diagnostic values.Results The MRI grades of group A were normal,and the MRI grades compared between the group B and C were significantly different(P<0.05).The AQP-MRI voxels of the globus pallidus,substantia nigra and thalamus in groups C and B were 51.33±5.39,8.22±0.43,35.32±5.18,and 61.04±9.67,10.56±0.89,44.79±6.92,which were significantly lower than those in group A(78.09±5.02,17.83±4.21,62.98±5.77),and those in group C were significantly lower than those in group B,the differences were statistically significant(P<0.05).The AQP-ADC and ADC of group C and B were 0.41±0.09,0.60±0.02 and 0.27±0.03,0.47±0.04,which were significantly higher than those of group A(0.21±0.04,0.35±0.05),and group C were significantly higher than that of group B,the differences were statistically significant(P<0.05).In group C of 24 cases,Spearman correlation analysis showed that AQP-MRI AQP-ADC,ADC were positively correlated with Alzheimer's disease(r=0.612,0.672,P<0.05).ROC curve analysis showed that the areas under the curve of AQP-MRI AQP-ADC,ADC for the diagnosis of Alzheimer's disease were 0.813 and 0.766,respectively.Conclusion Alzheimer's disease patients are often accompanied by changes in brain function MRI grades,and are often accompanied by AQP-MRI AQP-ADC,ADC elevations.AQP-MRI quantitative parameters have good value in the diagnosis of Alzheimer's disease.
