100例血清人类免疫缺陷病毒抗体阴性的男男性行为者肛门人乳头瘤病毒感染情况分析 被引量:3

摘要 目的对天津地区100例血清人类免疫缺陷病毒(HIV)阴性的男男性行为(MSM)者肛门人乳头瘤病毒(HPV)的感染情况进行分析。方法通过男同性恋专用社交软件及天津同性恋公益组织(非政府组织)及天津医科大学皮肤性病科门诊在天津市纳入HIV阴性且否认既往存在HPV感染的MSM者,采集MSM者肛门脱落细胞,采用PCR-反向斑点杂交技术检测MSM者肛门脱落细胞HPV,运用人乳头瘤病毒基因分型检测试剂盒分析HPV基因型。通过纸质问卷调查收集100例MSM的社会人口统计学和性行为信息,采用多变量Logistic回归模型分析MSM者肛门HPV感染的危险因素。结果100例MSM者中肛门HPV阳性58例(58%),MSM者肛门中最常见高危HPV基因型是HPV16、HPV33和HPV51,低危型HPV基因型检出率较高的是HPV6、HPV11。学历低于本科、第一次肛交年龄<18岁、不坚持使用避孕套和最近半年有肛交史是MSM者肛门感染任何型别的HPV的危险因素(AOR分别为6.865、10.368、4.041、23.686;95%CI分别为1.329~35.459、2.553~42.109、1.315~12.416、3.075~182.439;P均<0.05)。结论天津市HIV阴性的MSM者肛门HPV阳性率较高,肛门HPV感染以高危型HPV为主。学历低于本科、第一次肛交年龄<18岁、不坚持使用避孕套和最近半年有肛交史的MSM者应注意预防HPV感染。 Objective To analyze anal human papillomavirus(HPV)infection in 100 human immunodeficiency virus(HIV)-negative men who had sex with men(MSM)in Tianjin.Methods MSM in Tianjin who were HIV-negative and denied previous HPV infection were enrolled through the gay's social software and Tianjin Homosexual Public Welfare Organization(non-governmental organization)and the department of Dermatology and Venereology in Tianjin Medical University General Hospital.Anal exfoliated cells were collected from MSM,and the HPV genotype was analyzed using the HPV genotyping test kit.Information of the socio-demographics and sexual behavior of 100 MSM was collected through a paper-based questionnaire,and the risk factors for anal HPV infection in MSM were analyzed using multivariate Logistic regression models.Results Among 100 MSM,58(58%)were anal HPV positive.HPV16,33 and 51 were the most common high-risk HPV genotypes in the anus of MSM.HPV6 and HPV11 were the most common low-risk HPV genotypes.The risk factors for anal infection of any type of HPV in MSM were education lower than bachelor's degree,the age of first anal intercourse<18 years old,non-insisting on condom use and a history of anal sex in the last six months(AOR=6.865,10.368,4.041,and 23.686,respectively;95%CI were 1.329-35.459,2.553-42.109,1.315-12.416,and 3.075-182.439,respectively;all P<0.05).Conclusions The positive rate of anal HPV is higher in HIV negative MSM in Tianjin,and the anal HPV infection is mainly high-risk HPV.MSM who have education less than bachelor's degree,whose age of first anal sex is less than 18 years old,and who do not insist on using condoms and have a history of anal sex in the last half year should pay attention to the prevention of HPV infection.
