摘要
目的观察类风湿关节炎(RA)患者外周血CD4^(+)T、CD8^(+)T、NK细胞共信号分子CD160的表达变化,并探讨其临床意义。方法RA患者80例(RA组)、系统性红斑狼疮(SLE)患者60例(SLE组)及健康志愿者60例(健康组),RA组、SLE组住院期间(健康志愿者于同期)抽取外周静脉血。采用流式细胞术测算CD4^(+)T、CD8^(+)T、NK细胞CD160阳性表达率,采用Pearson相关分析法分析CD4^(+)T、CD8^(+)T、NK细胞上阳性表达率与RA患者临床病理参数的相关性。结果与健康组比较,RA组CD8^(+)T细胞CD160阳性表达率降低,NK细胞CD160阳性表达率升高(P均<0.05)。CD8^(+)T细胞上阳性表达率与RA患者28个关节疾病活动(DAS28)评分、血清抗环瓜氨酸肽抗体(CCP)水平、红细胞沉降率(ESR)、血清C-反应蛋白(CRP)水平呈负相关(r分别为-0.21、-0.28、-0.07、-0.07;P均<0.05)。NK细胞上阳性表达率与RA患者DAS28评分、血清CCP水平、ESR、血清CRP及IL-6水平呈正相关(r分别为0.13、0.346、0.331、0.14、0.18;P均<0.05)。RA患者中疾病低度活动度20例,中、高度活动度各30例,CD8^(+)T细胞CD160阳性表达率分别为7.82%±6.70%、5.22%±2.81%、2.67%±2.26%,NK细胞CD160阳性表达率分别为1.84%±2.17%、4.67%±1.77%、9.82%±6.70%,随疾病活动度升高,RA患者CD8^(+)T细胞CD160阳性表达率降低,NK细胞CD160阳性表达率升高(P均<0.05)。结论RA患者外周血CD8^(+)T细胞CD160表达降低、NK细胞CD160表达升高。CD8^(+)T细胞CD160表达降低、NK细胞CD160表达升高与RA疾病活动度有关。CD8^(+)T、NK细胞上CD160可能参与了RA的发生发展。
Objective To observe expression of co-signal molecule CD160 in peripheral blood CD4^(+)T cells,CD8^(+)T cells and NK cells of rheumatoid arthritis(RA)patients,and to explore its clinical significance.Methods Eighty cases of RA patients(RA group),60 cases of systemic lupus erythematosus(SLE)patients(SLE group),and 60 cases of healthy volunteers(health group)were selected.The peripheral venous blood were extracted from all patients during hospital stay,and blood collection was completed in the healthy volunteers in the same period.The positive expression rates of CD160 in CD4^(+)T cells,CD8^(+)T cells,and NK cells were measured by flow cytometry.The correlations of positive expression rates in CD4^(+)T cells,CD8^(+)T cells and NK cells with clinical pathological parameters in patients with RA were analyzed by Pearson correlation analysis.Results Compared with health group,the positive expression rate of CD160 decreased in CD8^(+)T cells and increased in NK cells in the RA group(both P<0.05).The positive expression rate of CD160 was negatively correlated with disease activity(DAS28)score,serum anti-cyclic citrullinated peptide antibody(CCP)level,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)level in CD8^(+)T cells of RA patients(r=-0.21,-0.2,-0.07,-0.07;all P<0.05),and was positively correlated with DAS28 score,CCP level,ESR,CRP level and in terleukin 6(IL-6)level in NK cells of RA patients(r=0.13,0.346,0.331,0.14,0.18;all P<0.05).In the RA group,low disease activity was found in 20 cases,moderate and high disease activity in 30 cases;the positive expression rates of CD160 were 7.82%±6.70%,5.22%±2.81%,and 2.67%±2.26%in CD8^(+)T cells;the positive expression rates of CD160 were 1.84%±2.17%,4.67%±1.77%,and 9.82%±6.70%in NK cells.As disease activity increased,the positive expression rate of CD160 was reduced in CD8^(+)T cells and increased in NK cells in patients with RA(both P<0.05).Conclusions The expression of CD160 decreases in CD8^(+)T cells and increases in NK cells in peripheral blood of RA patients.Disease activity of RA is related to the down-regulated expression of CD160 in CD8^(+)T cells and up-regulated expression of CD160 in NK cells.CD160 in CD8^(+)T cells and NK cells may be involved in the development and progression of RA.
