彩超检测中晚期妊娠胎儿UA及MCA血流频谱在宫内窘迫预测中的价值 被引量:4

摘要 目的:探讨彩超检测中晚期妊娠胎儿UA及MCA血流频谱在宫内窘迫预测中的价值。方法:选取2019年6月-2021年6月在定西市人民医院产科产检并分娩的中晚期妊娠孕妇50例正常妊娠者作为对照组,50例伴有宫内窘迫症状者作为观察组,两组均采用彩色多普勒超声进行检测,检测指标包括胎儿脐动脉(UA)和大脑中动脉(MCA)的血流搏动指数(PI)、阻力指数(RI)、收缩期与舒张期流速比(S/D)等,对两组的结果进行对比。结果:两组胎儿UA血流频谱比较,观察组胎儿PI、RI、S/D等指标均高于对照组,组间差异有统计学意义(P<0.05);两组胎儿MCA血流频谱比较,观察组胎儿PI、RI、S/D等指标均低于对照组,组间差异有统计学意义(P<0.05);分析两组胎儿彩色多普勒超声监测结果发现,孕28、32、36周时,对照组胎儿UA及MCA血流频谱的各指标均呈略微下降趋势;而观察组胎儿UA血流频谱的各指标则呈现出升高趋势;MCA血流频谱各指标呈较为明显的下降趋势。结论:通过采用彩色多普勒超声检测中晚期妊娠胎儿UA及MCA血流频谱可为临床上预测胎儿宫内窘迫提供参考,值得推广使用。 Objective:To explore the value of color Doppler ultrasound detection of fetal UA and MCA blood flow spectrum in the prediction of intrauterine distress in the middle and late pregnancy.Method:From June 2019 to June 2021,50 normal pregnant women in the middle and late stages of pregnancy who underwent obstetric examination and delivery in Dingxi People’s Hospital were selected as the control group,and 50 patients with intrauterine distress symptoms were selected as the observation group.Color Doppler ultrasound was used for detection in both groups.The detection indicators included the pulsatility index(PI),resistance index(RI),systolic to diastolic flow ratio(S/D)of fetal umbilical artery(UA)and middle cerebral artery(MCA),the results of the two groups were compared.Result:Compared with the fetal UA blood flow spectrum between the two groups,the fetal PI,RI,S/D in the observation group were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Compared with the MCA blood flow spectrum of the two groups,the fetal PI,RI,S/D in the observation group were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05).By analyzing the results of fetal color Doppler ultrasound monitoring in the two groups,it was found that at 28,32 and 36 weeks of gestation,each index of fetal UA and MCA blood flow spectrum in the control group showed a slight downward trend;while each index of fetal UA blood flow spectrum in the observation group showed an increasing trend;the MCA blood flow spectrum showed a relatively obvious downward trend.Conclusion:Using color Doppler ultrasonography to detect fetal UA and MCA blood flow spectrum in middle and late pregnancy can provide a reference for clinical prediction of fetal distress,and it is worthy of popularization.
