探讨湿热痹阻证类风湿关节炎与血小板参数的相关性

Discuss of Correlation between Rheumatoid Arthritis of Damp Heat Blockage Syndrome and Platelet Parameters

目的:研究湿热痹阻证类风湿关节炎(rheumatoid arthritis,RA)与血小板参数的相关性。方法:选取2018年1月-2021年10月福建中医药大学附属人民医院风湿病科住院湿热痹阻证RA患者60例作为RA组。选取60例同时期于本院健康体检者作为健康组。所有受试对象取静脉血进行血常规检测,进行DAS28-ESR评分。比较两组血小板参数与DAS28-ESR评分,分析DAS28-ESR评分与血小板计数(platelet count,PLT)、血小板平均体积(mean platelet volume,MPV)、血小板压积(platelet thrombocytocrit,PCT)、血小板分布宽度(platelet distribution width,PDW)及大血小板比率(platelet-large cell ratio,P-LCR)的相关性。结果:RA组PLT、PCT均高于健康组,MPV、PDW、P-LCR均低于健康组,RA组DAS28-ESR评分高于健康组(P<0.05)。DAS28-ESR评分与PLT、PCT均呈正相关,DAS28-ESR评分与MPV、P-LCR均呈负相关(P<0.05)。结论:湿热痹阻证RA患者血小板参数发生变化,其中PLT、PCT水平与RA活动性呈正相关,与MPV、P-LCR呈负相关,血小板参数对临床中评估湿热痹阻证RA病情活动具有一定意义,在临床中应动态监测。

Objective:To explore the correlation between rheumatoid arthritis (RA) of damp heat blockage syndrome and platelet parameters.Method:From January 2018 to October 2021,60 RA patients of damp heat blockage syndrome in the Rheumatology Department of the People’s Hospital Affiliated to Fujian University of Chinese Medicine were selected as RA group.60 cases of physical examination in our hospital at the same time were selected as the healthy group.Venous blood was taken from all subjects for routine blood test and they taken DAS28-ESR score.The blood routine parameters and DAS28-ESR scores were compared between the two groups.The correlations among DAS28-ESR scores and platelet count (PLT),mean platelet volume (MPV),platelet thrombocytocrit (PCT),platelet distribution width (PDW) and platelet-large cell ratio (P-LCR) were analyzed.Result:PLT and PCT in RA group were higher than those in healthy group,MPV,PDW and P-LCR were lower than those in healthy group,and DAS28-ESR score in RA group was higher than that in healthy group (P<0.05).DAS28-ESR score was positively correlated with PLT and PCT,and DAS28-ESR score was negatively correlated with MPV and P-LCR (P<0.05).Conclusion:The platelet parameters of RA patients of damp heat blockage syndrome has changed.The levels of PLT and PCT are positively correlated with RA activity,and it is negatively correlated with MPV and P-LCR.The platelet parameters have certain significance in evaluating the disease activity of RA with damp heat blockage syndrome in clinical,and should be dynamically monitored in clinical.
