射频消融术治疗心脏瓣膜疾病合并房颤的围手术期护理配合体会

Perioperative Nursing Experience of Radiofrequency Ablation in the Treatment of Heart Valve Disease Complicated with Atrial Fibrillation

目的:探讨射频消融术治疗心脏瓣膜疾病合并房颤的围手术期护理配合效果。方法:回顾性分析2018年2月-2020年2月在中南大学湘雅二医院心脏外科行二尖瓣置换手术、合并房颤并同时接受双极射频消融治疗的患者100例为本次研究对象,分析术前、术中护理及术后随访的护理配合对患者手术成功率、手术时间和住院时间及手术护理配合前后患者不良情绪的影响。结果:100例均手术成功,成功率100%,手术时间为45~90 min,平均(61.26±11.43)min;住院时间为7~15 d,平均(11.42±1.37)d。术后跟踪随访24个月,均为窦性心律。手术护理配合后的焦虑自评量表(SAS)和抑郁自评量表(SDS)均低于手术护理配合前,差异均有统计学意义(P<0.05)。结论:在射频消融术治疗心脏瓣膜疾病合并房颤的围手术期中采用护理配合可有效降低患者术后SAS和SDS,改善不良情绪,对于提高患者术后生存质量具有积极作用。

Objective:To explore the perioperative nursing cooperation and application effect of radiofrequency ablation for heart valve disease combined with atrial fibrillation.Method:Retrospective analysis of 100 patients who underwent mitral valve replacement surgery complicated with atrial fibrillation and received bipolar radiofrequency ablation at the same time were selected in the Department of Cardiac Surgery of the Second Xiangya Hospital of Central South University from February 2018 to February 2020.The impact of preoperative and intraoperative care and postoperative follow-up nursing coordination on the success rate of surgery,operative time,length of stay and patients’negative emotions before and after operation and nursing cooperation.Result:All 100 cases were successfully operated,with a success rate of 100%.The operation time was 45-90 minutes,with an average of(61.26±11.43)minutes;the hospitalization time was 7-15 days,with an average of(11.42±1.37)days.All patients were followed up for 24 months after operation,and all patients were in sinus rhythm.It was found that the self-rating anxiety scale(SAS)and self-rating depression scale(SDS)after the operation and nursing cooperation were lower than those before the operation and nursing cooperation,and the differences were statistically significant(P<0.05).Conclusion:The use of nursing cooperation during the perioperative period of radiofrequency ablation for heart valve disease combined with atrial fibrillation can effectively reduce the postoperative SAS and SDS of patients and improve their psychological status,which has a positive effect on improving the postoperative survival quality of patients.
