膝关节置换术患者术后应用冰袋冷敷联合四黄水蜜方外敷的效果分析

Analysis of Application Effect of Ice Pack Cold Compress Combined with Sihuang Shuimi Formula for External Application after Knee Arthroplasty

目的:观察采用膝关节置换术的患者术后应用冰袋冷敷联合四黄水蜜方外敷的效果。方法:选取2020年1月-2021年6月82例在福建中医药大学附属人民医院进行膝关节置换术的患者。根据随机数字表法将其分为试验组和对照组,各41例。对照组采用常规护理和术后冰袋冷敷,试验组在对照组基础上采用四黄水蜜方外敷。比较两组术后12、24、48 h切口引流量,术后24、48、72 h患肢肿胀程度,术后6、24、48、72 h疼痛度及术后3周膝关节情况。结果:两组术后12、24 h切口引流量比较差异均无统计学意义(P>0.05),试验组术后48 h切口引流量明显少于对照组(P<0.05)。术后48、72 h,试验组患肢肿胀程度均明显小于对照组(P<0.05)。术后48、72 h,试验组视觉模拟评分法(VAS)评分均低于对照组(P<0.05)。术后3周,试验组膝关节屈曲度及膝关节伸直度均大于对照组(P<0.05),WOMAC评分低于对照组(P<0.05)。结论:膝关节置换术后采用冰袋冷敷配合四黄水蜜方外敷可促进肢体肿胀缓解,同时还能够减轻切口疼痛度并促进其膝关节功能改善,值得临床应用。

Objective:To observe the application effect of ice pack cold compress combined with Sihuang Shuimi Formula for external application after knee arthroplasty.Method:A total of 82 patients who underwent knee arthroplasty in People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from January 2020 to June 2021 were selected.According to the random number table method,they were divided into experimental group and control group,with 41 cases in each group.The control group was given routine nursing and postoperative ice pack cold compress,while the experimental group was given Sihuang Shuimi Formula for external application on the basis of the control group.The incision drainage volume at 12,24 and 48 hours after operation,the swelling degree of the affected limb at 24,48 and 72 hours after operation,the pain degree at 6,24,48,72 hours after operation and the knee joint condition at 3 weeks after operation were compared between the two groups.Result:There were no significant differences in the incision drainage between the two groups at 12 h and 24 h after operation (P>0.05),and the incision drainage at 48 h after operation in the experimental group was significantly less than that in the control group (P<0.05).At 48 and 72 h after operation,the swelling degree of the affected limbs in the experimental group was significantly smaller than that in the control group (P<0.05).At 48 h and 72 h after operation,the visual analogue scale (VAS) score of the experimental group were lower than those of the control group (P<0.05).Three weeks after operation,the knee flexion and knee extension in the experimental group were larger than those in the control group (P<0.05),and the WOMAC score was lower than that in the control group (P<0.05).Conclusion:The use of ice pack cold compress combined with Sihuang Shuimi Formula for external application after knee arthroplasty can promote the relief of limb swelling,and reduce the incision pain and promote the improvement of knee joint function,which is worthy of clinical application.
