微创浅筋膜松解术治疗急性腰扭伤即时效应的临床随机对照研究

A Randomized Controlled Clinical Study of the Immediate Effect of Minimally Invasive Superficial Fascia Release in the Treatment of Acute Lumbar Sprain

目的:观察微创浅筋膜松解术治疗急性腰扭伤的即时临床效果。方法:选取2019年10月-2021年10月于青岛大学医学院松山医院接受治疗的急性腰扭伤患者150例,按照随机数字表法分为A、B两组,各75例。A组完成73例,B组完成72例。A组应用微创浅筋膜松解术1次,B组给予双氯芬酸钠双释放肠溶胶囊75 mg口服1次。分别记录两组在治疗前和治疗后即刻、0.5、24 h的疼痛视觉模拟量表(visual analogue scale,VAS)、腰椎活动度(range of movement,ROM)、腰椎功能障碍问卷(Roland-Morris disability questionnaire,RMDQ)评分,评估两组在治疗前后的临床效果。结果:A组治疗后3个时间节点(即刻、0.5 h、24 h)的VAS、RMDQ及ROM评分均明显低于治疗前(P<0.05);B组治疗后3个时间节点的VAS、RMDQ及ROM评分与治疗前比较差异无统计学意义(P>0.05)。A组总有效率97.26%、B组总有效率18.06%,A组高于B组,差异有统计学意义(P<0.01)。结论:微创浅筋膜松解术治疗急性腰扭伤有显著的效果,能快速减轻患者疼痛,恢复腰椎活动功能,值得临床推广。

Objective:To observe the immediate clinical effect of minimally invasive superficial fascia release in the treatment of acute lumbar sprain.Method:A total of 150 patients from October 2019 to October 2021 in Songshan Hospital,Qingdao University Medical College were selected and divided into the group A and group B by random table method,75 cases in each group.A total of 73 cases in group A and 72 cases in group B were completed.Group A received minimally invasive superficial fascia release once,and group B received Diclofenac Sodium Dual Release Enteric-soluble capsule 75 mg once.The visual analogue scale (VAS),range of movement (ROM) and Roland-Morris disability questionnaire (RMDQ) scores of group A and group B were recorded before and three time nodes (immediately,0.5 h,24 h) after treatment respectively,evaluate the clinical efficacy of the two groups before and after treatment.Result:The VAS,RMDQ and ROM scores of group A at three time nodes (immediately,0.5 h,24 h) after treatment were significantly lower than those before treatment (P<0.05).The VAS,RMDQ and ROM scores of group B at 3 time nodes after treatment showed no significant differences compared with those before treatment (P>0.05).The total effective rate was 97.26% in group A and 18.06% in group B,the group A was higher than group B,and the difference was statistically significant (P<0.01).Conclusion:Minimally invasive superficial fascia release in the treatment of acute lumbar sprain has significant curative effect,can quickly improve the pain of patients,restore lumbar activity function,worthy of clinical promotion.
