大黄[庶虫]虫丸治疗糖尿病肾脏疾病的效果

Effect of Dahuang Zhechong Pills in the Treatment of Diabetic Kidney Disease

目的:观察大黄[庶虫]虫丸治疗糖尿病肾脏疾病(DKD)的效果。方法:选择2018年8月-2021年6月就诊于江苏省中医院的64例DKD患者。随机将其分为对照组(n=38)和治疗组(n=26)。共脱落12例患者,最终实际分析病例对照组34例,治疗组18例。两组均予以基础治疗,治疗组在上述基础上给予大黄[庶虫]虫丸。比较两组临床疗效,治疗前及治疗4、8、12周尿微量白蛋白/尿肌酐(UACR),治疗前及治疗12周血清肌酐(Scr)、BUN、eGFR、糖化血红蛋白(Hb A1c)、血清白蛋白(ALB)、血红蛋白(Hb)及不良反应。结果:治疗12周后,治疗组西医疗效明显优于对照组(P<0.05),中医疗效明显优于对照组(P<0.05)。两组治疗前与治疗4、8、12周UACR比较差异均无统计学意义(P>0.05),对照组治疗8周与治疗前相比UACR增加(Z=-2.768,P=0.006)。治疗12周,两组BUN、Scr、eGFR比较差异均无统计学意义(P>0.05)。两组治疗前及治疗12周Hb A1c、ALB、Hb比较差异均无统计学意义(P>0.05),治疗组治疗12周ALB比治疗前高(t=-2.745,P=0.014)。两组治疗过程中均未发生明显不良反应,肝功能指标无明显异常。结论:大黄[庶虫]虫丸治疗DKD具有较好的临床效果,能有效改善中医症候积分,稳定肾功能,提高血清ALB水平,无明显不良反应。

Objective:To investigate the effect of Dahuang Zhechong Pills in the treatment of diabetic kidney disease(DKD).Method:A total of 64 patients with DKD who were hospitalized in Jiangsu Province Hospital of Chinese Medicine from August 2018 to June 2021 were selected.They were randomly divided into control group(n=38)and treatment group(n=26).A total of 12 patients fell out,and finally the actual analysis of 34 cases in the control group and 18 cases in the treatment group.Both groups were given basic treatment,and the treatment group was given Dahuang Zhechong Pills on the above basis.The clinical effects,urinary microalbumin/urinary creatinine(UACR)before treatment and 4,8 and 12 weeks of treatment,serum creatinine(Scr),BUN,eGFR,glycosylated hemoglobin(HbA1c),serum albumin(ALB),hemoglobin(Hb)before and 12 weeks of treatment,adverse reactions were compared between the two groups.Result:After 12 weeks of treatment,the western medical curative effect of the treatment group was significantly better than that of the control group(P<0.05),and the curative effect of traditional Chinese medicine was significantly better than that of the control group(P<0.05).There were no significant differences in UACR between the two groups before treatment and 4,8 and 12 weeks of treatment(P>0.05),UACR in the control group increased 8 weeks of treatment compared with that before treatment(Z=-2.768,P=0.006).12 weeks of treatment,there were no significant differences in BUN,Scr and eGFR between the two groups(P>0.05).There were no significant differences in HbA1c,ALB and Hb between the two groups before treatment and 12 weeks of treatment(P>0.05),12 weeks of treatment,the ALB in the treatment group was higher than that before treatment(t=-2.745,P=0.014).There were no obvious adverse reactions and no abnormal liver function indexes in the two groups during the treatment process.Conclusion:Dahuang Zhechong Pills has a good clinical effect in the treatment of DKD.It can effectively improve the symptom score of traditional Chinese medicine,stabilize renal function,and increase the level of serum ALB without obvious adverse reactions.
