MMP-3、Anti-CCP抗体、RF在类风湿关节炎患者中的表达及与免疫功能相关性

Expression of MMP-3,Anti-CCP Antibody and RF in Patients with Rheumatoid Arthritis and Their Correlation with Immune Function

目的:探讨血清基质金属蛋白酶-3(MMP-3)、抗环状瓜氨酸(CCP)抗体、类风湿因子(RF)在类风湿关节炎患者中的表达及其临床意义,并分析其与患者免疫功能的相关性。方法:回顾性收集2021年1-12月咸宁市第一人民医院收治的类风湿关节炎患者105例作为观察组,同期收集体检中心健康体检者100例作为对照组。比较组间血清MMP-3、Anti-CCP抗体、RF及免疫球蛋白IgA、IgM、IgG的变化情况,分析其诊断价值,采用Pearson相关性分析MMP-3、Anti-CCP抗体、RF与免疫球蛋白IgA、IgM、IgG的相关性。结果:观察组MMP-3、Anti-CCP抗体、RF水平明显高于对照组(P<0.05),3项联合检测诊断类风湿关节炎的曲线下面积(AUC)为0.840,敏感度和特异度分别为82.0%、70.1%,均高于MMP-3、Anti-CCP抗体或RF单项检测,进一步提高了诊断效能。活动组患者的MMP-3、Anti-CCP抗体、RF及IgA、IgM、IgG水平明显高于缓解组,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示MMP-3、Anti-CCP抗体、RF与免疫球蛋白IgA、IgM、IgG呈正相关(P<0.05)。结论:血清MMP-3、Anti-CCP抗体、RF在类风湿关节炎患者中明显升高,且与患者免疫功能密切相关,联合检测有助于疾病的早期诊断。

Objective:To investigate the expression and clinical significance of serum matrix metalloproteinase-3(MMP-3),anti cyclic citrullinate peptide(CCP)antibody and rheumatoid factor(RF)in patients with rheumatoid arthritis,and to analyze the correlation between the expression and immune function of patients.Method:A total of 105 patients with rheumatoid arthritis admitted to Xianning City First People’s Hospital from January to December 2021 were retrospectively selected as the observation group,and 100 healthy subjects in the physical examination center were selected as the control group.The changes of serum MMP-3,Anti-CCP antibody and RF as well as immunoglobulin IgA,IgM and IgG between the two groups were compared,and their diagnostic value was analyzed.Pearson correlation was used to analyze the correlation between MMP-3,Anti-CCP antibody and RF and immunoglobulin IgA,IgM and IgG.Result:The levels of MMP-3,Anti-CCP antibody and RF in the observation group were significantly higher than the control group(P<0.05).The area under the curve(AUC)of the three combined tests for the diagnosis of rheumatoid arthritis was 0.840,and the sensitivity and specificity were 82.0%and 70.1%,respectively,which were higher than MMP-3,Anti-CCP antibody or RF single test,further improved the diagnostic efficiency.The levels of MMP-3,Anti-CCP antibody,RF,IgA,IgM and IgG in the dynamic group were significantly higher than those in the remission group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that MMP-3,Anti-CCP antibody and RF were positively correlated with IgA,IgM and IgG(P<0.05).Conclusion:Serum MMP-3,Anti-CCP antibody and RF are significantly increased in patients with rheumatoid arthritis,and are closely related to the immune function of patients.Combined detection is helpful for the early diagnosis of the disease.
