中药外洗联合蓝光照射治疗新生儿高胆红素血症的临床效果

Clinical effect of external washing of traditional Chinese medicine combined with blue light irradiation on neonatal hyperbilirubinemia

目的 观察中药外洗联合蓝光照射治疗新生儿高胆红素血症的临床效果。方法 选取2020年6—12月茂名市人民医院收治的高胆红素血症的患儿80例,依照随机分组对照原则分为观察组和对照组,各40例。对照组患儿接受蓝光照射治疗,观察组在对照组基础上加用中药外洗,2组疗程均为5 d。比较2组治疗效果,治疗前后相关临床指标[血清神经元特异性烯醇化酶(NSE)、间接胆红素(UCB)、β微球蛋白(β2-MG)]及胆红素相关指标[血清胆红素(TSB)、直接胆红素(DB)、经皮黄疸指数(TCB)]变化。结果 观察组患儿治疗总有效率为95.00%,高于对照组的77.50%(χ^(2)=5.165,P=0.023);治疗5 d后,2组NSE、USB、β-MG水平及TSB、DB和TCB水平均较治疗前降低,且观察组低于对照组(P均<0.01)。结论 中药外洗联合蓝光照射治疗新生儿高胆红素血症的临床效果显著,患儿临床指标与胆红素相关指标均明显改善,值得临床推广应用。

Objective To observe the clinical effect of external washing of traditional Chinese medicine combined with blue light irradiation in the treatment of neonatal hyperbilirubinemia. Methods 80 children with hyperbilirubinemia treated in Maoming People’s Hospital from June to December 2020 were selected. According to the principle of random grouping and control,they were divided into observation group and control group,with 40 cases in each group. The children in the control group were treated with blue light irradiation,and the observation group was treated with external washing of traditional Chinese medicine on the basis of the control group. The course of treatment of both groups was 5 days. The therapeutic effects of the two groups were compared,and the changes of relevant clinical indexes( NSE,UCB,β-MG) and bilirubin related indexes( TSB,DB,TCB) before and after treatment were compared. Results The total effective rate of the observation group was 95. 00%,higher than 77. 50% of the control group( χ^(2)~2= 5. 165,P = 0. 023). After 5 days of treatment,the levels of NSE,USB,β-MG,TSB,DB and TCB in the two groups were lower than those before treatment,and the levels in the observation group were lower than those in the control group( P < 0. 01). Conclusion External washing of traditional Chinese medicine combined with blue light irradiation has a significant clinical effect on neonatal hyperbilirubinemia. The clinical indexes and bilirubin related indexes of children were significantly improved,which is worthy of clinical application.
