摘要 目的:探讨3D打印导板在经皮微创通道螺钉治疗老年骨盆脆性骨折(FFP)中的应用效果。方法:回顾性分析2020年1月-2021年7月于萍乡市人民医院进行经皮微创通道螺钉手术治疗的51例老年FFP患者的临床资料。依据是否使用3D打印导板将其分为常规组(n=22)与导板组(n=29),常规组实施常规经皮微创通道螺钉治疗,导板组基于常规组使用3D打印导板治疗。比较两组螺钉置入状况;比较两组透视次数、手术时间、术中出血量及切口长度;评估两组骨折复位状况;比较两组并发症发生情况。结果:导板组螺钉置入状况、骨折复位状况均优于常规组,透视次数、术中出血量均少于常规组,手术时间、切口长度均短于常规组,差异均有统计学意义(P<0.05);导板组并发症发生率为3.45%,低于常规组的22.73%,差异有统计学意义(P<0.05)。结论:在老年FFP患者经皮微创通道螺钉治疗中使用3D打印导板可提升骨折复位质量,改善螺钉置入状况,手术切口小,术中出血少,降低术后并发症发生率。 Objective:To investigate the application effect of 3D printing guide plate in the treatment of elderly fragility fractures of the pelvis(FFP)with percutaneous minimally invasive channel screws.Method:The clinical data of 51 elderly patients with FFP treated by percutaneous minimally invasive channel screw surgery in Pingxiang People’s Hospital from January 2020 to July 2021 were retrospectively analyzed.They were divided into conventional group(n=22)and guide group(n=29)according to whether 3D printed guide plates were used.The conventional group received conventional percutaneous minimally invasive channel screw treatment,and the guide group received 3D printed guide plates based on the conventional group.Screw placement was compared between the two groups.The number of fluoroscopy,operation time,intraoperative blood loss and incision length were compared between the two groups.The fracture reduction status of the two groups was evaluated.The incidence of complications was compared between the two groups.Result:The screw placement and fracture reduction of the guide group were better than those of the conventional group,the number of fluoroscopy and intraoperative blood loss were less than those of the conventional group,the operation time and incision length were shorter than those of the conventional group,the differences were statistically significant(P<0.05).The complication rate of the guide group was 3.45%,which was significantly lower than 22.73%of the conventional group(P<0.05).Conclusion:In the minimally invasive percutaneous channel screw treatment of patients with FFP,the use of 3D printed guide plate can improve the quality of fracture reduction,improve screw placement,small surgical incision,less intraoperative bleeding,and reduce the incidence of postoperative complications.
