回阳保心汤治疗慢性心力衰竭合并持续性心房纤颤86例疗效观察

Effect observation of Huiyang Baoxin decoction in treating 86 cases of chronic heart failure combined with persistent atrial fibrillation

目的:观察回阳保心汤治疗慢性心力衰竭合并持续性心房纤颤的临床疗效。方法:纳入慢性心力衰竭合并持续性心房纤颤患者172例,随机分为治疗组和对照组各86例,对照组给予基础抗心衰治疗,治疗组在对照组的基础上加回阳保心汤,每日1剂,两组均治疗8周,疗程结束比较两组治疗前后氨基末端脑钠肽前体(N-terminal pro-B-type Natriuretic Peptide,NTproBNP)、C-反应蛋白(C-reactive Protein,CRP)、心脏左心室舒张末期内径(Left Ventricular End Diastolic Diameter,LVEDD)、左室收缩末期容积(Left Ventricular End-systolic Volume,LVESV)、左室射血分数(Left Ventricular Ejection Fraction,LVEF)、全血高切黏度、全血低切黏度、红细胞比容、纤维蛋白原值的变化。结果:治疗组临床疗效优于对照组(P<0.05),两组治疗后NT-proBNP、CRP均降低(P<0.05),心功能较治疗前改善(P<0.05),治疗组更显著(P<0.05);治疗后,两组患者的全血高切黏度、全血低切黏度、红细胞比容、纤维蛋白原值均降低(P<0.01),且治疗组下降更显著,差异有统计学意义(P<0.01)。结论:回阳保心汤治疗慢性心力衰竭合并持续性心房纤颤临床疗效确切,可改善心脏功能,降低NT-proBNP、CRP水平,降低血液黏稠度,无明显不良反应,值得临床推广应用。

Objective:To observe the clinical effect of Huiyang Baoxin decoction(回阳保心汤)on chronic heart failure combined with persistent atrial fibrillation.Methods:172 patients with chronic heart failure combined with persistent AF were randomly divided into the treatment group and the control group,with 86 cases in each group.The control group was treated with basic anti heart failure,and the treatment group received Huiyang Baoxin decoction on the basis of the treatment of the control group.The course of treatment was 8 weeks,one dose per day.The results of NT-proBNP,CRP,LVEDD,LVESV and LVEF were compared at the end of treatment.Results:The clinical effect of the treatment group was better than that of the control group(P<0.05).After treatment,NT-proBNP and CRP were decreased(P<0.05),and the cardiac function was improved(P<0.05),and the effect of the treatment group was more significant(P<0.05).After treatment,the whole blood viscosity,whole blood low shear viscosity,RBC specific volume and fibrinogen value of the two groups were decreased(P<0.01),and the difference was statistically significant(P<0.01).Conclusion:Huiyang Baoxin decoction has a definite clinical effect on chronic heart failure combined with persistent atrial fibrillation,it can improve the heart function,reduce NT-proBNP and CRP level,reduce blood viscosity,and it has no obvious adverse reactions,which is worth popularizing and applying.
