自拟扶伤消毒饮联合常规抗感染治疗对四肢骨折术后患者血清炎症因子及凝血功能的影响

Effect of Self-designed Fushang Xiaodu decoction combined with conventional anti-infection treatment on serum inflammatory factors and coagulation function of patients with limb fracture after operation

目的:观察自拟扶伤消毒饮联合常规抗感染治疗对四肢骨折术后患者血清炎症因子及凝血功能的影响。方法:选取本院106例行手术治疗的四肢骨折患者,利用数字表法随机分为两组,各53例,对照组术后接受常规抗感染治疗,试验组于其基础上予以自拟扶伤消毒饮,比较两组术后血清炎症因子[C反应蛋白(C-reactive Protein,CRP)、降钙素原(Procalcitonin,PCT)]、白细胞计数(White Blood Cell,WBC)、中性粒细胞计数(Neutrophil Count,NEUT)、凝血功能[纤维蛋白原(Fibrinogen,FIB)、红细胞沉降率、血小板计数(Platelet,Plt)]、体温变化及感染发生率。结果:试验组术后第3天、第7天CRP、PCT、WBC、NEUT明显低于对照组(P<0.05);试验组术后第3天、第7天FIB、Plt及红细胞沉降率明显低于对照组(P<0.05);试验组术后第3天、第7天体温明显低于对照组(P<0.05);试验组术后感染发生率明显低于对照组(P<0.05)。结论:自拟扶伤消毒饮联合常规抗感染治疗可有效地改善四肢骨折患者术后炎症反应与高凝状态,提高机体免疫力,降低感染风险。

Objective:To observe the effect of self-designed Fushang Xiaodu decoction(自拟扶伤消毒饮)combined with conventional anti-infection treatment on serum inflammatory factors and coagulation function of patients with limb fracture after operation.Methods:A total of 106 patients with limb fracture undergoing surgical treatment in Chongqing Jiangjin Chinese Medicine Hospital were selected and randomly divided into two groups by using random number table method,with 53 patients in each group.The control group received routine anti-infection treatment after surgery,and the experimental group was given Self-designed Fushang Xiaodu decoction combined with conventional anti-infection treatment.Serum inflammatory factors[C-reactive protein(CRP),procalcitonin(PCT)],white blood cell count(WBC),neutrophil count(NEUT),coagulation function[fibrinogen(FIB),erythrocyte sedimentation rate(ESR),platelet count(Plt)],temperature change and infection rate were compared between the two groups.Results:The CRP,PCT,WBC and NEUT of the experimental group were significantly lower than those of the control group on the 3rd and 7th day after operation(P<0.05).The FIB,Plt and ESR of the experimental group were significantly lower than those of the control group on the 3rd and 7th day after operation(P<0.05).The body temperature of the experimental group was significantly lower than that of the control group on the 3rd and 7th day after operation(P<0.05).The incidence of postoperative infection in experimental group was significantly lower than that in control group(P<0.05).Conclusion:Self-designed Fushang Xiaodu decoction combined with conventional anti-infection treatment can effectively improve postoperative inflammatory response and hypercoagulability in patients with limb fractures,it can improve immunity and reduce the risk of infection.
