固本止崩汤加减治疗脾虚型无排卵型异常子宫出血的疗效探讨

Effect of modified Guben Zhibeng decoction on anovulatory abnormal uterine bleeding with spleen deficiency

目的:探讨固本止崩汤加减治疗脾虚型无排卵型异常子宫内出血的疗效。方法:于广州市番禺区中医院2018年6月-2019年12月接受治疗的脾虚型无排卵型异常子宫出血患者临床资料中随机选取83例进行回顾性分析,根据随机分组法分为观察组(42例)与对照组(41例),对照组单纯使用宫腔镜进行治疗,在此基础上观察组加用固本止崩汤加减治疗,观察宫腔镜术前、术后治疗情况。结果:观察组阴道止血时间<3 d、4~7 d、8~10 d的患者相比对照组较少(P>0.05),>10 d的患者相比对照组较少(P<0.05)。在治疗总有效率方面,观察组优于治疗组,(P<0.05)。观察组不良反应发生率低于对照组,(P<0.05)。观察组中医证候积分下降幅度高于对照组(P<0.05)。与对照组比较,观察组PT、APPT等指标水平下降更明显,Fib水平升高更明显(P<0.05)。结论:固本止崩汤加减治疗配合宫腔镜治疗脾虚型无排卵型异常子宫出血方面均有显著疗效,固本止崩汤的不良反应发生率更低,具有推广价值。

Objective:To explore the curative effect of Guben Zhibeng decoction(固本止痛汤)on anovulatory abnormal intrauterine bleeding of spleen deficiency type.Methods:83 patients with spleen deficiency anovulatory abnormal uterine bleeding treated in our hospital from June 2018 to December 2019 were randomly selected for retrospective analysis,According to the random grouping method,they were divided into observation group(42 cases)and control group(41 cases).The control group was treated with hysteroscopy alone.On this basis,the observation group was treated with Guben Zhibeng decoction,and the treatment before and after hysteroscopy was observed.Results:There were fewer patients in the observation group with vaginal hemostasis time<3D,4-7d and 8-10d compared with the control group(P>0.05)there were fewer patients in the observation group than in the control group(P<0.05).In terms of the total effective rate of treatment,the observation group was better than the treatment group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).The decline of TCM syndrome score in the observation group was higher than that in the control group(P<0.05).Compared with the control group,the levels of Pt,appt and Fib in the observation group decreased more significantly,and the level of FIB increased more significantly(P<0.05).Conclusion:The addition and subtraction treatment of Guben Zhibeng decoction combined with hysteroscopy has significant curative effect in the treatment of anovulatory abnormal uterine bleeding of spleen deficiency type,and the incidence of adverse reactions of Guben Zhibeng decoction is lower,which has popularization value.
