腰肌劳损患者应用中医正骨推拿联合浮针治疗效果分析

Efficacy of TCM bone-setting massage plus floating acupuncture on lumbar muscle strain

目的:分析腰肌劳损患者应用中医正骨推拿联合浮针治疗的临床效果。方法:以随机数字表法将2018年9月-2020年9月湖南省中医药研究院附属医院脊柱骨肿瘤科收治的78例腰肌劳损患者均分为对照组和研究组。对照组给予浮针治疗,研究组在对照组浮针治疗的基础上联合中医正骨推拿治疗。分别以中文版Oswestry功能障碍指数(Chinese Oswestry Disability Index,CODI)和改良日本骨科协会腰痛评分表(Modified Japanese Orthopaedic Association Scores,M-JOA)对患者腰部功能和症状、体征严重程度进行评估,比较两组血清5-羟色胺和炎症因子(肿瘤坏死因子-α、白细胞介素-6和C反应蛋白)水平、腰椎活动度。结果:与治疗前相比,两组治疗后的疼痛、单项功能、个人综合功能评分和CODI均下降,研究组均显著低于对照组,两组差异有统计学意义(P<0.05)。研究组症状、体征轻度比例高于对照组,重度比例低于对照组,两组差异具有统计学意义(P<0.05)。与治疗前相比,两组治疗后的M-JOA评分均下降,研究组低于对照组;腰部耐力时间均增加,研究组长于对照组;腰部活动度评分均下降,研究组低于对照组,两组差异具有统计学意义(P<0.05)。治疗后两组前屈,左、右侧屈,左、右侧旋均显著提高,与对照组相比,研究组显著更高(P<0.05)。结论:中医正骨推拿联合浮针治疗的腰肌劳损患者腰部功能提高,症状、体征有效缓解,疼痛减轻,炎症因子指标改善,患者腰椎活动度改善,临床效果显著。

Objective:To analyze efficacy of TCM bone-setting massage plus floating acupuncture on lumbar muscle strain.Methods:From September 2018 to September 2020,a total of 78 cases were divided into 2 groups.The control group was given floating acupuncture,and the study group was treated by TCM bone-setting massage more.Results:Compared with before treatment,pain,single function,personal comprehensive function score and CODI were decreased in both groups after treatment,and in the study group were significantly lower than the control group.The difference between the two groups was statistically significant(P<0.05).The proportion of mild symptoms and signs in the study group was higher than the control group,and the proportion of severe symptoms was lower than that in the control group(P<0.05).Compared with before treatment,the M-JOA score in both groups were decreased after treatment,and the study group was lower than the control group;the waist endurance time was increased,and the study group was longer than the control group;the lumbar mobility scores were decreased,and in the study group was lower than the control group(P<0.05).After treatment,the forward flexion,left and right flexion,and left and right rotation were significantly improved in the two groups,and in the study group was significantly higher(P<0.05).Conclusion:TCM bone-setting massage plus floating acupuncture has significant clinical effects in the treatment of lumbar muscle strain,which can improve the patient’s lumbar function,relieve symptoms and signs,relieve pain,and improve inflammatory factors and lumbar spine mobility.
