北部空军招飞医学选拔鼻黏膜苍白水肿者血清IgE检测阳性率分析

Positive rates of serum IgE in pilot candidates with pale nasal mucosawith edemadetected during medical selection for pilot recruitment in Northern Air Force

目的探讨在招飞医学选拔中对鼻黏膜苍白水肿者进行血清IgE检测,确诊变应性鼻炎,实现精准招飞的可行性。方法选择2017年1月—2021年3月鲁豫两省和东北三省自愿报考空军飞行学员的12838名高三学生为应招对象,检出有鼻黏膜苍白水肿体征者,并对其行血清IgE检测。并将地域分离的山东、河南两省候检者的检出结果和辽宁、吉林、黑龙江三省进行对比。结果在12838候选人中共检出鼻黏膜苍白水肿者777人(6.05%),其中鲁豫两省8047人中检出524人(6.51%);东北三省4791人中检出253人(5.28%)。两地域检出率差异有统计学意义(χ^(2)=7.79,P<0.01)。鼻黏膜苍白水肿者777人中血清IgE阳性517人(66.54%),其中鲁豫两省524人中IgE阳性351人(66.98%);东北三省253人中IgE阳性166人(65.61%)。两地域IgE阳性率比较差异无统计学意义。结论在招飞医学选拔中对可疑AR被检者可通过血清IgE的检测进行确诊,减少AR的漏诊和误淘,为做出体检结论提供精准依据,是安全、方便、可行的。

Objective To explore the feasibility of allergic rhinitis(AR)being diagnosed by detecting serum levels of IgE among pilot candidates with pale nasal mucosa with edemain order to make the recruitment precise.Methods Between January 2017 and March 2021,12838 graduating students of high schools in Shandong,Henan,Heilongjiang,Jilin and Liaoning provinces who had applied for eligibility for the Air Force f light cadets were selected as the subjects.Those with pale nasal mucosa with edema were identified,whose levels of serum IgE were detected.In addition,the results ofdetection were compared between candidates from Shandong and Henan and those from Heilongjiang,Jilin and Liaoning.Results A total of 777(6.05%)students with pale nasal mucosa with edema were detected from these candidates,524 of whom(6.51%)were from Shandong and Henan,and 253(5.28%)were from Heilongjiang,Jilin and Liaoning.There was statistically significant difference in the rate of detection between the two regions(χ^(2)=7.79,P<0.01).Among the 777 subjects with pale nasal mucosa with edema,517(66.54%)were IgE positive,351 of whom(66.98%)were from Shandong and Henan,and 166(65.61%)were from Heilongjiang,Jilin and Liaoning.There was no statistically significant difference in IgE positiverates between the two regions.Conclusion It is safe,user-friendly and feasible to diagnose AR through serum IgE detectionin suspected AR subjects during medical selection for pilot recruitment to reduce the rate of missed diagnosis and misdiagnosis of AR.
