飞行员幽门螺杆菌感染对胃蛋白酶原及胃泌素的影响

摘要 目的探讨飞行员人群中幽门螺杆菌感染的检出率及其对胃蛋白酶原Ⅰ与Ⅱ、胃泌素-17分泌的影响。方法选择在空军特色医学中心进行改装体检的飞行员进行呼气试验及胃蛋白酶原Ⅰ与Ⅱ、胃泌素-17检测,根据呼气试验结果将研究对象分为2组(呼气试验阳性组、呼气试验阴性组),应用两独立样本t检验对比分析2组人员检测结果的差异。结果根据纳入排除标准,共入组116人,其中呼气试验阳性组46人(39.66%),阴性组70人(60.34%),2组胃蛋白酶原Ⅰ分别为(72.15±19.68)μg/L与(49.17±13.13)μg/L,差异有统计学意义(t=7.549,P<0.001);2组胃蛋白酶原Ⅱ分别为(14.00±4.77)μg/L与(6.92±2.33)μg/L,差异有统计学意义(t=10.654,P<0.001);2组胃蛋白酶原Ⅰ与Ⅱ的比值分别为5.42±1.29、7.40±1.71,差异有统计学意义(t=-6.705,P<0.001)。2组胃泌素-17分别为(54.08±14.76)pg/ml、(62.48±29.16)pg/ml,差异无统计学意义(t=-1.805,P>0.05)。结论改装体检飞行员人群中幽门螺杆菌感染率较低。在改装体检飞行员中,幽门螺杆菌感染使胃蛋白酶I及胃蛋白酶原Ⅱ升高、胃蛋白酶原Ⅰ/Ⅱ比值下降,但其对于胃泌素-17分泌无显著影响。 Objective To investigate the prevalence of Helicobacter pylori infection in pilots and its effect on thesecretion of pepsinogen(PGI,PGII)and gastrin-17.Methods Pilots who were undergoing physical examination in our center before f lying higher performance aircraft were selected for 13 C-urea breath test,pepsinogen IandIItest,and gastrin-17 test.According to the results of the breath test,the subjects were divided into two groups(positive and negativebreath test groups)before two-sample independent T-tests were used to analyze the difference between the two groups.Results By the criteria for inclusion and exclusion,a total of 116 pilots were enrolled,46 of whom were in the positive breath test group(39.66%),and the rest were in the negative group(60.34%).The levels of PGI inthe two groups were(72.15±19.68)μg/L and(49.17±13.13)μg/L,respectively,so the difference was statistically significant(t=7.549,P<0.001).The levels of PGⅡ in thetwo groups were(14.00±4.77)μg/L and(6.92±2.33)μg/L,respectively,and the difference was statistically significant(t=10.654,P<0.001).The ratios of pepsinogen Ⅰ to Ⅱ(PGR)in the two groups were 5.42±1.29 and 7.40±1.71,respectively,so the difference was of statistical significance(t=-6.705,P<0.001).The gastrin-17levels of the two groups were(54.08±14.76)pg/ml and(62.48±29.16)pg/ml,respectively,so the difference was not statistically significant between the two groups(t=-1.805,P>0.05).Conclusion The infection rate of Helicobacter pylori among these pilots for higher performance aircraft is relatively low.Helicobacter pylori infection can increase levels of pepsinogenⅠandⅡ,decrease the ratio of pepsinogen Ⅰ to pepsinogen Ⅱ,but has no significant effect on the secretion of gastrin-17.
