应对方式在老年肺癌化疗患者心理痛苦和生活质量间的中介作用

Mediating Effect of Coping Style on Psychological Distress and Quality of Life in Elderly Patients with Lung Cancer Undergoing Chemotherapy

目的分析应对方式在老年肺癌化疗患者心理痛苦及生活质量的中介作用,为改善患者的生活质量提供参考。方法选取2021年7月至2022年1月西部战区总医院211例老年肺癌化疗患者进行调查,分析应对方式在心理痛苦和生活质量间的中介作用。结果老年肺癌化疗患者生活质量总分为(94.65±16.51)分,生活质量与心理痛苦、屈服呈负相关(P<0.05),与面对、回避呈正相关(P<0.05);心理痛苦与面对、回避呈负相关(P<0.05),与屈服呈正相关(P<0.05)。面对、屈服在心理痛苦和生活质量间的中介效应为-0.031、-0.122,分别占总效应的6.9%、23.9%。结论老年肺癌化疗患者心理痛苦和应对方式均与生活质量密切相关,面对和屈服在心理痛苦与生活质量间起部分中介作用,将心理痛苦和应对方式进行综合干预,有助于改善老年肺癌化疗患者的生活质量。

Objective To explore the mediating effect of coping style on psychological distress and quality of life in elderly lung cancer patients undergoing chemotherapy.Methods A total of 211 elderly lung cancer patients undergoing chemotherapy in the General Hospital of the Western Theater Command from July 2021 to January 2022 were investigated by using general information questionnaire,Distress Thermometer(DT),Medical Coping Modes Questionnaire(MCMQ)and Functional Assessment of Cancer Therapy-Lung Cancer(FACT-L).The mediating effect of coping style on psychological distress and quality of life was analyzed.Results The total score of FACT-L was(94.65±16.51).Quality of life was negatively correlated with psychological distress and resigned acceptance,while positively correlated with confrontation and avoidance(P<0.05).Psychological distress was negatively correlated with confrontation and avoidance,while positively correlated with resigned acceptance(P<0.05).The mediating effects of confrontation and resigned acceptance on psychological distress and quality of life were-0.031 and-0.122,accounting for 6.9%and 23.9%of the total effects,respectively.Conclusion The psychological distress and coping style of elderly lung cancer patients undergoing chemotherapy were closely related to their quality of life.Confrontation and resigned acceptance played a partial mediating role between psychological distress and quality of life.Comprehensive intervention of psychological distress and coping style can improve the quality of life of elderly lung cancer patients undergoing chemotherapy.
