军事人员精索静脉曲张彩色多普勒超声诊断价值及分析

Diagnostic value and analysis of color Doppler ultrasonography for varicocele in military personnel

目的探讨彩色多普勒超声在军事人员精索静脉曲张中的应用价值及分析,以便能更好为军事人员健康保障提供参考数据。方法选取与空军特色医学中心阴囊不适就诊行阴囊睾丸精索静脉超声检查的患者5723例,其中军事人员4558例为观察组,普通人员1165例作为对照组。观察军事人员精索静脉曲张发生率、分级情况及测量平静呼吸时曲张内径。结果军事人员超声检出精索静脉曲张1562例(34.2%),普通人员超声检出精索静脉曲张305例(26.1%)。军人亚临床型占比为3%;临床型以Ⅰ度为主占比为50%,较普通人员Ⅰ度发生率明显增高(P<0.05);Ⅱ度占比为22%;Ⅲ度占比为25%,平均内径[(3.7±0.4)mm]较普通人员组Ⅲ度曲张内径[(3.3±0.3)mm]明显增宽(P<0.05)。普通人员亚临床型占比为6.6%;临床型Ⅰ度占比为31%;Ⅱ度占比为27%;Ⅲ度占比为35.4%。结论超声对军事人员精索静脉曲张诊断有着重要的价值,可以清楚的诊断分度,为临床提供数据支持。军事人员精索静脉曲张发生率较普通人员明显增高,曲张程度以I度占比最高,而当发生精索静脉曲张皿度程度要较普通人员更显著。

Objective To explore the application value and analysis of color Doppler ultrasonography in varicocele of military personnel,so as to provide reference data for health protection of military personnel.Methods A total of 5723 patients with scrotal discomfort in our hospital were selected for ultrasonography of scrotal,testicular and spermatic veins,including 4558 military personnel as the observation group and 1165 general personnel as the control group.The incidence and grade of varicocele in military personnel were observed and the inner diameter of varicocele was measured during calm breathing.Results Varicocele was detected by ultrasound in 1562(34.2%)military personnel and 305(26.1%)general personnel.The subclinical type accounted for 3%of servicemen.In clinical type,Ⅰdegree accounted for 50%,the incidence of Ⅰ degree is significantly higher than ordinary people(P<0.05).The proportion ofⅡdegree was 22%.The proportion ofⅢdegree was 25%,the inner diameter of theⅢdegree[(3.7±0.4)mm]was significantly wider than that of the ordinary group[(3.3±0.3)mm](P<0.05).The proportion of subclinical type was 6.6%.The proportion of clinical typeⅠdegree was 31%.The proportion ofⅡdegree was 27%.The proportion ofⅢdegree was 35.4%.Conclusion The degree of varicocele in military personnel can be clearly diagnosed by ultrasonography,and the incidence of varicocele in military personnel is significantly higher than that in ordinary personnel.
