等速肌力训练对脑卒中偏瘫患者下肢运动功能恢复的疗效观察 被引量:2

摘要 目的 观察对脑卒中偏瘫患者进行等速肌力训练对其下肢运动功能及日常生活活动能力的影响。方法 将2020年11月-2021年8月收治的脑卒中偏瘫患者。按随机数字法随机分为常规治疗组和等速肌力训练对照组。治疗组进行常规康复治疗,等速肌力训练对照组在常规康复治疗基础上增加等速肌力训练,治疗8周后,比较两组患者患肢运动功能改善后的Fugl-Meyer评分、改良巴氏指数评定量表评分、Berg平衡积分。结果 治疗开始时,两组患者的Fugl-Meyer评分、改良巴氏指数评定量表评分、Berg平衡评分相比,无明显差异,P>0.05。8周治疗后,等速肌力训练组患者的Fugl-Meyer评分、改良巴氏指数评定量表评分、Berg平衡评分均高于常规组患者,P<0.05。等速肌力训练组患者接受训练至患肢运动功能开始改善的时间及住院的时间均短于常规组患者,P<0.05。结论 对脑卒中后偏瘫患者进行等速肌力训练的效果显著,能够有效提高患者偏瘫侧下肢肌力及患侧肢体膝关节稳定性,且进一步提高患者的平衡功能、步行能力和日常生活能力,促进其康复。 Objective To observe the effect of isokinetic muscle training on lower limb motor function and daily living ability of stroke patients with hemiplegia.Methods Stroke patients with hemiplegia admitted to the Rehabilitation Department of Harbin 242 Hospital in Heilongjiang Province from November 2020 to August 2021 were enrolled. Patients in group A underwent isokinetic muscle training on the basis of routine rehabilitation. The patients were randomly divided into routine treatment group and isokinetic muscle training control group by random number method. Patients in the treatment group received routine rehabilitation treatment. After 8 weeks of isokinetic muscle training, fugl-Meyer score, modified Barberis index rating Scale score and Berg balance score were compared between the two groups after improved motor function of affected limbs.Results At the beginning of treatment, there were no significant differences in fugl-Meyer score, modified Pap index rating Scale score and Berg balance score between 2 groups(P>0.05). After 8 weeks of treatment, fugl-Meyer score, modified Pap scale score and Berg balance score of isokinetic muscle training group were higher than those of conventional group, P < 0.05. The time from training to improvement of motor function of affected limb and hospitalization time in isokinetic muscle training group were shorter than those in conventional group, P<0.05. Conclusions Isokinetic muscle strength training has a significant effect on patients with post-stroke hemiplegia, which can improve the muscle strength of the affected lower limb and the stability of the knee joint, further improve the balance function, walking ability and daily living ability of patients, and promote their rehabilitation.
