摘要
目的 分析慢性阻塞性肺病患者拔管后序贯经鼻高流量氧疗的效果。方法选择2019年4月—2020年4月该院收治的92例慢性阻塞性肺病患者开展本次研究,根据治疗方式的不同分成对照组与观察组,二组各有患者46例,对照组患者于拔管后实施普通鼻导管氧疗,观察组于拔管后实施序贯经鼻高流量氧疗,比较二组患者的治疗效果。结果 治疗后观察组的呼气峰值流速(PEF)、第一秒用力呼气容积(FEV1)、第一秒用力呼气容积/用力肺活量(FEV1/FVC)、氧合指数均高于对照组,差异有统计学意义(P<0.05);观察组的血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、氢离子浓度指数(pH)血气指标改善程度均显著优于对照组,差异有统计学意义(P<0.05);观察组患者的ICU停留时间、治疗耐受度评分均比对照组低,差异有统计学意义(P<0.05);观察组的再插管率、谵妄发生率、误吸发生率、28天死亡率与对照组比较,差异无统计学意义(P>0.05)。结论 慢性阻塞性肺病患者在拔管之后进行序贯经鼻高流量氧疗,能获得更佳疗效,显著改善患者的肺功能与血气指标,缩短患者ICU停留时间,提高其治疗耐受度。
Objective To analyze the effect of sequential transnasal hyperflow oxygen therapy after extubation in patients with chronic obstructive pulmonary disease.Methods From April 2019 to April 2020,ninety-two patients with chronic obstructive pulmonary disease were selected,They were divided into control groups and observation groups according to the different treatment methods,There were forty-six c ases in each group,Normal nasal catheter oxygen therapy was performed in the control group after extubation,The observation group performed sequential transnasal hyperflow oxygen therapy after extubation,To compare the therapeutic effect of two oxygen therapy methods.Results The peak expiratory flow rate(PEF),forced expiratory volume(FEV1),FEV1/forced vital capacity(FVC) and oxygenation index of the observation group were higher than those of the control group,Contrastive difference(P0.05).Conclusion Patients with COPD underwent sequential transnasal hyperflow oxygen therapy after extubation,To get better results,Significantly improved lung function and blood gas,shorten ICU patient stay,To improve their treatment tolerance.
