艾灸联合中医情志护理路径干预对慢性乙型肝炎合并肝纤维化患者的应用效果

Effect of Moxibustion Combined with Traditional Chinese Medicine Emotional Nursing Pathway in the Treatment of Patients with Chronic Hepatitis B Complicated with Liver Fibrosis

目的探讨艾灸联合中医情志护理路径干预对慢性乙型肝炎合并肝纤维化患者的应用效果。方法将本院2019年1月至2021年1月收治的80例慢性乙型肝炎肝纤维化患者随机分为对照组和观察组,每组40例,对照组给予常规护理干预,观察组给予艾灸联合中医情志护理路径干预,两组均干预3个月,治疗前、治疗3个月进行中医症候评分,采用医疗应对模式问卷(MCMQ)进行患者应对方式评价,采用Morisky量表评价两组患者治疗依从性,使用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评价患者抑郁及焦虑改善情况,采用简明健康状况量表(SF-36)评价患者生活质量变化情况。结果观察组治疗3个月胁肋疼痛、脘闷腹胀、情志抑郁、乏力评分均明显低于治疗前及对照组治疗后,P<0.05。观察组治疗3个月消极应对评分明显低于治疗前及对照组治疗后,P<0.05,积极应对评分明显高于治疗前及对照组治疗后,P<0.05。观察组和对照组治疗前治疗依从性比较,差异无统计学意义,P>0.05;观察组治疗3个月治疗依从性明显高于治疗前及对照组治疗后,P<0.05。观察组治疗3个月HAMD评分、HAMA评分明显低于治疗前及对照组治疗后,P<0.05,SF-36评分明显高于治疗前及对照组治疗后,P<0.05。结论艾灸联合中医情志护理路径干预在慢性乙型肝炎合并肝纤维化的应用有助于改善患者对疾病的应对方式,提高治疗依从性,减轻负性情绪,提高生活质量。

Objective To explore the application effect of moxibustion combined with TCM emotional nursing pathway intervention on patients with chronic hepatitis B complicated with liver fibrosis.Methods A total of 80 patients with chronic hepatitis B liver fibrosis admitted to our hospital from January 2019 to January 2021 were randomly divided into the control group and the observation group,40 cases in each group.The control group was given routine nursing intervention,while the observation group was given moxibustion combined with TCM emotional nursing pathway intervention.Both groups were given intervention for 3 months.Before treatment and 3 months after treatment,TCM symptom score was performed.The medical coping model questionnaire(MCMQ)was used to evaluate the coping style of the patients,the Morisky scale was used to evaluate the treatment compliance of the two groups,and the Hamilton depression scale(HAMD)and the Hamilton anxiety scale(HAMA)were used to evaluate the depression and anxiety improvement of the patients.The SF-36 quality of life scale was used to evaluate the changes of patients'quality of life.Results After 3 months of treatment,the scores of flank pain,abdominal distension,emotional depression and fatigue in the observation group were significantly lower than those before treatment and those in the control group after treatment,P<0.05.The negative coping score in the observation group after 3 months of treatment was significantly lower than before treatment and after treatment in the control group,P<0.05,and the positive coping score in the observation group was significantly higher than that before treatment and after treatment in the control group,P<0.05.There was no significant difference in the treatment compliance between the observation group and the control group before treatment,P>0.05.The treatment compliance in the observation group after 3 months of treatment was significantly higher than that before treatment and after treatment in the control group,P<0.05.The HAMD score and HAMA score of the observation group after 3 months of treatment were significantly lower than those before treatment and after treatment in the control group,P<0.05,and the SF-36 score was significantly higher than those before treatment and after treatment in the control group,P<0.05.Conclusion The application of moxibustion combined with TCM emotional nursing pathway intervention in chronic hepatitis B complicated with liver fibrosis can help to improve patients'coping style,improve treatment compliance,reduce negative emotions,and improve quality of life.
