清胰降脂汤治疗高脂血症性胰腺炎的疗效观察及对胰腺CT的影响

Effect of Qingyi Jiangzhi Decoction on Hyperlipidemic Pancreatitis and its Effect on Pancreatic CT

目的:探讨清胰降脂汤联合西医常规治疗高脂血症性胰腺炎(HLAP)的临床疗效及对胰腺CT的影响。方法:将符合纳入标准的HLAP患者60例,按照随机数字表法分为观察组和对照组,各30例。对照组采用西医常规治疗,观察组采用清胰降脂汤联合西医常规治疗,两组均治疗1周。观察两组患者症状、体征缓解时间,比较治疗前后血清甘油三酯(TG)、血清淀粉酶(AMY)、尿淀粉酶(UAMY)、血清脂肪酶(LPS)、血清C反应蛋白(CRP)、胰腺CT变化;比较改良CT严重指数评分(MCTSI评分),比较临床疗效。结果:治疗后,观察组的总有效率为93.33%,明显高于对照组(73.33%)(P<0.05)。观察组腹胀及腹痛缓解时间、肠鸣音恢复时间、MCTSI评分均显著低于对照组(P<0.05);治疗后两组患者TAG、AMY、UAMY、LPS均明显下降(P<0.05),且观察组均明显低于对照组(P<0.05)。结论:清胰降脂方联合西医常规治疗HLAP临床疗效显著,可有效改善临床症状,促进胃肠功能恢复,降低TAG,降低MCTSI评分,改善胰腺CT表现,值得在临床上推广。

Objective:To investigate the clinical efficacy of Qingyi Jiangzhi formula combined with western medicine in the treatment of hyperlipidemic pancreatitis (HLAP) and its effect on pancreatic CT. Methods:Sixty patients with HLAP who met the inclusion criteria were randomly divided into observation group and control group. The control group was treated with conventional western medicine, and the observation group was treated with Qingyi Jiangzhi Decoction Combined with conventional western medicine. Both groups were treated for 1 week. The relief time of symptoms and signs of the two groups were observed, and the changes of serum triglyceride (TG), serum amylase (AMY), urinary amylase (UAMY), serum lipase (LPS), serum C-reactive protein (CRP) and pancreatic CT were compared before and after treatment. The modified CT severity index score (mctsi score) was compared and the clinical efficacy was compared. Results: after treatment, the total effective rate of the observation group was 93.33%, which was significantly higher than that of the control group (73.33%) (P<0.05). The relief time of abdominal distension and abdominal pain, recovery time of bowel sounds and mctsi score in the observation group were significantly lower than those in the control group (P<0.05);After treatment, tag, Amy, uamy and LPS in the two groups were significantly lower than those in the previous group (P<0.05), and those in the observation group were significantly lower than those in the control group (P<0.05). Conclusion:Qingyi Jiangzhi Decoction Combined with conventional western medicine has significant clinical effect in the treatment of HLAP. It can effectively improve clinical symptoms, promote the recovery of gastrointestinal function, reduce tag, reduce mctsi score and improve the CT performance of pancreas. It is worthy to be popularized in clinic.
