急诊超声检查在腹部闭合性损伤内脏破裂出血诊断中的应用价值

Application Value of Emergency Ultrasound Examination in the Diagnosis of Visceral Rhexis Hemorrhage Caused by Blunt Abdominal Trauma

目的探讨急诊超声检查在腹部闭合性损伤(BAT)内脏破裂出血诊断中的应用价值。方法选取2019年8月至2020年8月医院收治的78例疑似BAT内脏破裂出血患者,均施以急诊超声检查,以手术病理检查结果作为金标准,分析急诊超声检查结果。结果经手术病理检查证实,肝破裂15例,肾破裂23例,脾破裂14例,胰破裂3例,腹膜后血肿9例,空腔脏器损伤6例,复合性损伤8例。急诊超声检查诊断符合率为92.31%(72/78),漏诊率为6.41%(5/78),误诊率为1.28%(1/78)。结论急诊超声检查是诊断BAT内脏破裂出血的有效检查方式,具有较高的诊断符合率,应用价值较高。

Objective The application value of emergency ultrasound examination in the diagnosis of visceral rhexis hemorrhage caused by blunt abdominal trauma(BAT)was explored.Methods 78 patients who were suspected with visceral rhexis hemorrhage caused by BAT and admitted to the hospital from August 2019 to August 2020 were selected.All of them underwent emergency ultrasound examination,and then taken the surgical pathological examination results as the diagnostic criteria,the results of emergency ultrasound examination were analyzed.Results The surgical pathological examination results showed that the cases of liver rupture,kidney rupture,spleen rupture,pancreatic rupture,retroperitoneal hematomas,cavity visceral injury and compound injuries were 15,23,14,3,9,6 and 8.The diagnostic coincidence rate,missed diagnosis rate and misdiagnosis rate of emergency ultrasound examination were 92.31%(72/78),6.41%(5/78)and 1.28%(1/78).Conclusion Emergency ultrasound examination is an effective method for the diagnosis of visceral rhexis hemorrhage caused by BAT,with a high diagnosis coincidence rate and high application value.
