摘要
目的探讨基于xSPECT/CT断层融合显像在儿童骨转移瘤中的诊断价值。方法对疑似骨转移瘤的152例患儿行全身骨显像、xSPECT/CT及SPECT/CT断层融合显像,比较和分析其xSPECT/CT和SPECT/CT断层融合图像以及核医学医师对2种方法的诊断信心,评估xSPECT/CT断层融合显像在儿童骨转移瘤中的诊断价值。结果xSPECT/CT断层融合显像诊断儿童骨转移瘤的敏感度、特异度、准确度分别为93.3%、91.5%、92.8%;SPECT/CT断层融合显像诊断儿童骨转移瘤的敏感度、特异度、准确度分别为85.7%、85.1%、85.5%。xSPECT/CT断层融合显像能提高核医学医师诊断信心且差异具有统计学意义[5(2~5)分vs.3(2~4)分,P<0.05]。结论xSPECT/CT断层融合显像能有效提高对儿童骨转移瘤的诊断效能,有助于早期明确患儿骨转移情况,从而进一步改善患儿预后。
Objective To investigate the diagnostic value of xSPECT/CT-based tomographic fusion imaging in children with bone metastases.Methods Clinical and imaging data of 152 children with suspected bone metastases examined in our department were collected.All children underwent whole-body bone imaging,xSPECT/CT,and SPECT/CT tomographic fusion imaging.xSPECT/CT and SPECT/CT tomographic fusion images were compared and analyzed to evaluate the diagnostic value of xSPECT/CT tomographic fusion imaging in children with bone metastases.The diagnostic confidence of nuclear medicine physicians of two methods was also compared.Results The sensitivity,specificity,and accuracy of xSPECT/CT tomographic fusion imaging in the diagnosis of bone metastases in children were 93.3%,91.5%,and 92.8%,respectively;the sensitivity,specificity,and accuracy of SPECT/CT tomographic fusion imaging in the diagnosis of bone metastases in children were 85.7%,85.1%,and 85.5%,respectively.xSPECT/CT tomographic fusion imaging could improve the diagnostic confidence of nuclear medicine physicians and the difference was statistically significant[5(2-5)vs.3(2-4),P<0.05].Conclusion xSPECT/CT tomographic fusion imaging can effectively enhance the diagnostic confidence and diagnostic efficacy of nuclear medicine physicians in the diagnosis of bone metastases in children,which can help to diagnose bone metastases in children at an early stage,and further improve their clinical prognosis.
