摘要
目的 分析2016-2020年信阳市某医院耐碳青霉烯肠杆菌(carbapenem-resistant enterobacteriaceae,CRE)分布特点及耐药性,为临床用药、制定医院抗菌谱等提供依据。方法 收集2016-2020年信阳市某医院各科室临床分离的肠杆菌科细菌,经常规分离培养、菌株鉴定及药敏试验获得目标菌株CRE,分析CRE分布特点及耐药性情况。结果 共分离14 280株肠杆菌科细菌,其中CRE菌株428株,占3.00%;CRE菌株中肺炎克雷伯菌占据首位,为58.41%;CRE菌株多集中于重症监护室(36.21%)、外科(25.70%)及呼吸内科(15.19%);CRE对阿卡米星、庆大霉素的药物敏感性尚可,对其他抗菌药物表现出高度耐药性。结论 2016-2020年信阳市某医院CRE分布较广泛,以肺炎克雷伯菌为主,且CRE多集中于重症监护室、外科与呼吸内科,对多数抗菌药物呈高度耐药性。
Objective To analyze the distribution characteristics and drug resistance of carbapenem-resistant enterobacteriaceae(CRE) in a hospital of Xinyang in 2016-2020, and provide evidence for clinical medication and hospital antibacterial spectrum.Methods Enterobacteriaceae clinically isolated from various departments in a hospital of Xinyang in 2016-2020 were collected;the target strain CRE was obtained by routine isolation and culture, strain identification and drug sensitivity test;the distribution characteristics and drug resistance of CRE were analyzed. Results There were 14 280 enterobacteriaceae isolated, of which 428 were CRE strains, accounting for 3.00%;of CRE strains, Klebsiella pneumoniae was most common(58.41%), and CRE strains were mostly concentrated in intensive care unit(36.21%), surgery department(25.70%), and respiratory medicine department(15.19%);CRE had a general drug sensitivity to acamicin and gentamicin, and showed high resistance to other antibiotics.Conclusion CRE is widely distributed in a hospital of Xinyang from 2016-2020, mainly Klebsiella pneumoniae, and CRE is mainly concentrated in intensive care unit, surgery department and respiratory medicine department, with high resistance to most antibiotics.
