从“治风先治血”论“风血同辨”治疗风牵偏视

Discussing on the treatment of wind-induced squint with "wind-blood differentia⁃ tion" from the theory of "expelling wind behind treating blood disorders"

本文基于“治风先治血”理论,对临床上治疗眼科疾病运用“风血同辨”法的合理性与必要性加以论述,并为其提供理论依据。全文从“病风”“病血”在病因上的关联性以及祛风药和理血药在配伍上的协同作用出发,阐明“风血同辨”的理论内涵,并提出风牵偏视的病机为“本虚”“失运”加之外受风邪,以“养血”“活血”为其治疗的侧重点,从而探究肝与脾的“风血关系”。依循和延伸“五轮学说”的同时,拓展了风牵偏视的中医治疗思路,以期为其他表现为风证或血证的眼科疾病提供临证参考。

Based on the theory of"expelling wind behind treating blood disorders",this paper discusses the rationality and necessity of using"wind-blood differentiation"in clinical treatment of ophthalmic diseases,and provides a theoretical basis for it.Starting from the correlation between"pathogenic wind"and"pathogenic blood"in etiology and the synergistic effect of wind expelling drugs and blood regulating drugs in compatibility,this paper expounds the theoretical connotation of"wind-blood differentiation"and puts forward that the pathogenesis of wind induced squint is"deficiency in origin""disfunction in transport"and external wind pathogens,thus leading to the focus of"nourishing blood"and"activating blood circulation"as the treatment,and exploring the"wind-blood relationship"between the corresponding viscera,liver and spleen.Following and extending the five wheel-theory,this paper also expands the treatment idea of wind-induced squint with Traditional Chinese Medicine,in order to provide clinical reference for other ophthalmic diseases with initial symptoms of wind or blood disease.
