摘要
对出院准备度的概念、老年病人出院准备度的评估工具及影响因素、老年病人出院准备度的干预方式进行综述,以期为临床老年病人出院准备度护理干预的完善和发展提供参考。
This study reviewed the concept of readiness for hospital discharge,assessment tools and influencing factors of readiness for hospital discharge of elderly patients,and intervention methods of readiness for hospital discharge in elderly patients,in order to provide reference for the improvement and development of readiness for hospital discharge nursing intervention in clinical elderly patients.
