强光疗治疗新生儿高胆红素血症的Meta分析 被引量:5

Meta-analysis of intensive phototherapy in treatment of neonatal hyperbilirubinemia

摘要 目的系统评价强光疗治疗新生儿高胆红素血症的临床疗效与安全性。方法全面检索中国知网、万方数据、PubMed、the Cochrane Library、Embase等国内外医学文献数据库,搜索有关我国强光疗治疗新生儿高胆红素血症的随机对照试验,检索时限为建库至2021年12月31日。并使用Cochrane偏倚风险评估工具对纳入的文献进行质量评价;使用Stata SE软件进行Meta分析。结果本研究共纳入12项随机对照试验(RCT),共2159例患儿。Meta分析结果显示:2组患儿在相同的治疗时间内,强光疗组患儿血清总胆红素(TBIL)下降更显著;强光疗组患儿黄疸消退时间较短;两组患儿不良反应发生率差异无统计学意义。结论和传统光疗比,强光疗可更为快速的降低血清总胆红素,缩短黄疸消退时间,治疗期间不增加不良反应发生率,有较重要的临床应用价值。 Objective To systematically evaluate the clinical efficacy and safety of intensive phototherapy in treatment of neonatal hyperbilirubinemia. Methods PubMed, the Cochrane Library, Embase, CNKI, WanFang Data, VIP etc.databases were searched to collect the randomized controlled trial(RCT) of intensive phototherapy in treatment of neonatal hyperbilirubinemia. The retrieval time was set from the establishment of the database to Dec. 31th, 2021. The quality assessment of included literature were performed according to the Cochrane standards, and Meta-analysis was conducted by Stata SE software. Results A total of 2159 newborns were included in 12 RCT. The results of Meta-analysis showed that the serum total bilirubin level in the intensive phototherapy group decreased more significantly in the same treatment time.Jaundice clearance time was shorter in the intensive phototherapy group. There was no significant difference in the incidence of adverse reactions between two groups. Conclusion Compared with traditional phototherapy, intensive phototherapy can reduce serum total bilirubin level more quickly,shorten the time of jaundice clearance, and does not increase the incidence of adverse reactions during the time of treatment, and has important application value.
