中年体检人群心肺运动试验中心血管事件发生情况及影响因素分析

Occurrence status quo and influencing factors of cardiovascular events during cardiopulmonary exercise testing in middle⁃aged population for physical examination

目的:探讨中年体检人群心肺运动试验过程中心血管事件发生情况及影响因素。方法:选取40~60岁体检的543人进行心肺运动试验,观察心肺运动负荷核心指标血压、心电图、最大摄氧量、无氧阈、二氧化碳通气当量、氧脉搏、呼吸储备、最大功率及博格评分(Borg Scale)指标,记录试验过程中心血管事件发生情况,分析心血管事件发生情况的影响因素。结果:543人心肺运动试验过程中发生心血管事件140人,发生率为25.78%,其中血压异常、ST段下降和心律失常的发生率分别为4.42%、20.81%和1.10%。Logistic回归分析显示,静息心电图和峰值收缩压是心血管事件发生的影响因素。结论:静息心电图和峰值收缩压是中年体检人群心肺运动试验过程中发生心血管事件的影响因素,可为心肺功能测评及健康指导提供依据。

Objective:To investigate the occurrence status quo and influencing factors of cardiovascular events during cardiopulmonary exercise testing in the middle⁃aged physical examination population.Methods:A total of 543 people aged 40⁃60 years old were selected for the cardiopulmonary exercise testing.Observe the cardiopulmonary exercise load's core indicators(blood pressure,electrocardiogram,maximum oxygen uptake,anaerobic threshold,carbon dioxide ventilation equivalent,oxygen pulse,respiratory reserve,maximum power,and Borg score);record the occurrence of cardiovascular events during the trial;and analyze the influencing factors of cardiovascular events.Results:Cardiovascular events occurred in 140 cases of 543 people during the exercise test,with an incidence rate of 25.78%.The incidences of abnormal blood pressure,ST segment decline,and arrhythmia were 4.42%,20.81%and 1.10%,respectively.Logistic regression analysis showed that Resting Electrocardiogram(ECG)and peak exercise systolic blood pressure(SBP)were the influencing factors of cardiovascular events.Conclusions:Resting ECG and peak SBP were the influencing factors of cardiovascular events during cardiopulmonary exercise tests in middle⁃aged population for physical examination.It could provide a basis for cardiopulmonary function evaluation and health guidance.
