宫腔镜下粘连分解术后放置COOK球囊治疗中重度IUA患者的效果

The Effect of COOK Balloon Placement in the Treatment of Moderate to Severe IUA Patients after Hysteroscopic Adhesion Decomposition

目的分析宫腔镜下粘连分解术后放置COOK球囊治疗中重度宫腔粘连(uterine adhesions,IUA)患者的效果。方法选取兴宁市妇幼保健计划生育服务中心2020年10月—2021年10月63例中重度IUA患者为研究对象,按照随机数表法分两组,观察组32例,对照组31例。两组患者均行宫腔镜下粘连分解术,对照组术后放置元宫型宫内节育器(Intrauterine devices,IUD),观察组放置COOK球囊。比较两组患者临床疗效、AFS评分改善率、月经情况及术后并发症。结果观察组临床治疗有效率为90.63%明显高于对照组的70.97%,组间比较差异有统计学意义(χ^(2)=3.946,P<0.05);观察组AFS评分改善率优于对照组,组间比较差异有统计学意义(P<0.05);观察组月经好转率及术后并发症总发生率分别为90.62%、9.38%,均明显优于对照组的67.74%、29.03%,两组比较差异有统计学意义(P<0.05)。结论宫腔镜下粘连分解术后放置COOK球囊治疗中重度IUA疗效明显,能够促进月经好转,改善AFS评分,且术后并发症发生率较低。

Objective To analyze the effect of COOK balloon placement in the treatment of moderate to severe intrauterine adhesions(IUA)after hysteroscopic adhesion decomposition.Methods A total of 63 patients with moderate to severe IUA in tXingning Maternal and Child Health and Family Planning Service Center from October 2020 to October 2021 were selected as the research objects,and divided into two groups according to the random number table method,with 32 cases in the observation group and 31 cases in the control group.Both groups of patients underwent hysteroscopic adhesion decomposition,the control group was placed with a uterine-type IUD after surgery,and the observation group was placed with a COOK balloon.The clinical efficacy,AFS score improvement rate,menstrual status and postoperative complications were compared between the two groups.Results The effective rate of clinical treatment in the observation group was 90.63%,which was significantly higher than that in the control group,which was 70.97%,and the difference was statistically significant(χ^(2)=3.946,P<0.05).The improvement rate of AFS score in the observation group was better than that in the control group,and the difference between the groups was statistically significant(P<0.05).The improvement rate of menstruation and the total incidence of postoperative complications in the observation group were 90.62% and 9.38%,which were significantly better than those in the control group,67.74% and 29.03%,with statistical significance between the two groups(P<0.05).Conclusion The COOK balloon placement after hysteroscopic adhesion decomposition has obvious curative effect in the treatment of moderate to severe IUA,which can promote the improvement of menstruation and improve the AFS score,and lower the incidence of postoperative complications.
