彩超联合检测多血管血流频谱对妊高征患者胎儿宫内窘迫的诊断效果评价 被引量:1

Evaluation of the Diagnostic Effect of Color Ultrasound Combined with Multi vessel Flow Spectroscopy on Intrauterine Distress in Fetus with Hyperemesis

摘要 目的分析彩超联合检测多血管血流频谱对妊高征患者胎儿宫内窘迫的诊断效果。方法从齐齐哈尔医学院附属第三医院2020年10月—2021年9月产检与分娩产妇中选择120例,按照诊断情况分组,60例为妊高征胎儿窘迫组(符合诊断标准),60例正常对照组(不符合窘迫诊断标准)。两组均进行彩超、多血管血流频谱检测,分析结果。结果妊高征胎儿窘迫组脐动脉(UA)血流动力学指标阻力指数(RI)、搏动指数(PI)、收缩期和舒张期流速比值(S/D)分别为(0.75±0.05)mm/s、(1.83±0.30)mm/s、(4.15±1.50)mmHg,高于正常对照组(0.60±0.08)mm/s、(1.06±0.28)mm/s、(2.64±0.60)mmHg,差异有统计学意义(t=12.316、14.534、7.240,P<0.001);大脑中动脉(MCA)血流动力学参数值低于对照组,差异有统计学意义(P<0.001);妊高征胎儿窘迫组静脉导管(DV)分流率高于对照组,差异有统计学意义(t=15.811,P<0.001);MCA检测灵敏度、特异度高于UA、DV检测,联合检测灵敏度、特异度高于单独检测,差异有统计学意义(P<0.05)。结论彩超联合检测多血管血流频谱能够提高诊断胎儿窘迫的精准性。 Objective To analyze the effectiveness of combined ultrasound detection of multivessel flow spectroscopy in the diagnosis of intrauterine distress in patients with hyperemesis.Methods 120 cases were selected from the labor and delivery in The Third Affiliated Hospital of Qiqihar Medical College from October 2020 to September 2021 and grouped according to the diagnosis,60 cases were in the fetal distress group for hyperemesis gravidarum(meeting the diagnostic criteria)and 60 cases were in the normal control group(not meeting the diagnostic criteria for distress).Ultrasound and multivessel flow spectroscopy were performed in both groups and the results were analyzed.Results umbilical artery(UA)hemodynamic parameters resistance index(RI),pulsatility index(PI),systolic and diastolic flow ratio(S/D)in PIH in fetal distress group were(0.75±0.05)mm/s,(1.83±0.30)mm/s,(4.15±1.50)mmHg were higher than normal controls(0.60±0.08)mm/s,(1.06±0.28)mm/s,(2.64±0.60)mmHg,the difference was statistically significant(12.316,14.534,7.240,P<0.001);middle cerebral artery(,MCA)hemodynamic parameters values were lower than controls,the difference was statistically significant(P<0.001);ductus venosus(DV)shunt rate was higher in the hyperemesis fetal distress group than in the control group,the difference was statistically significant(t=15.811,P<0.05);the sensitivity and specificity of the MCA assay were higher than those of the UA and DV assays,and the sensitivity and specificity of the combined assay were higher than those of the individual assays,the difference was statistically significant(P<0.05).Conclusion Combined ultrasound detection of multivessel flow spectra can improve the accuracy of the diagnosis of fetal distress.
