全髋与半髋关节置换术治疗老年股骨颈骨折对患者手术时间长短及髋关节功能的影响

Effect of Total and Hemi Hip Arthroplasty for Femoral Neck Fractures in Elderly Patients on the Length of Operation and Hip Function

目的对比分析治疗老年股骨颈骨折时运用全髋与半髋关节置换术对患者髋关节功能及手术时间的影响。方法选取2016年2月—2021年2月徐州市贾汪区第二人民医院老年股骨颈骨折患者28例为研究对象,依据电脑随机分组,采取半髋关节置换术治疗的14例为半髋组;行全髋关节置换的14例为全髋组。比较分析组间手术时间等临床指标、髋关节功能及术后并发症发生情况。结果全髋组术中出血量及手术时间分别为(307.46±45.13)mL、(83.25±9.73)min大于半髋组,差异有统计学意义(t=2.343,3.213,P<0.05);全髋组完全负重时间为(31.12±2.55)d,较半髋组更短,差异有统计学意义(t=15.029,P<0.05)。术后全髋组评分为(84.57±4.12)分,明显大于半髋组,差异有统计学意义(t=7.211,P<0.05)。全髋组并发症发生率0.00%显著低于半髋组35.71%,差异有统计学意义(P<0.05)。结论在治疗老年股骨颈骨折时运用全髋关节置换术虽手术时间和术中出血量高于半髋置换术,但全髋关节置换术术后并发症更少,安全性更高,髋关节功能能够尽快恢复。

Objective To compare and analyze the effect of using total hip versus hemi hip arthroplasty in the treatment of elderly femoral neck fractures on patients'hip function and operative time.Methods A total of 28 elderly patients with femoral neck fractures in the Second Peoples Hospital of Jiawang District in Xuzhou City from February 2016 to February 2021 were selected as the research objects.According to computer randomization,14 patients who received hemi-hip arthroplasty were selected as the hemi-hip group;14 cases of total hip arthroplasty were total hip group.Comparison and analysis of clinical indicators such as operation time,hip function and postoperative complications between groups.Results The intraoperative blood loss and operative time in the total hip group were(307.46±45.13)mL and(83.25±9.73)min respectively,and the differences were statistically significant(t=2.343,3.213,P<0.05).The full weight-bearing time in the total hip group was(31.12±2.55)d,which was shorter than that in the half-hip group,and the difference was statistically significant(t=15.029,P<0.05).The postoperative score of the total hip group was(84.57±4.12)points,which was significantly higher than that of the half hip group,and the difference was statistically significant(t=7.211,P<0.05).The complication rate of the total hip group was 0.00%,which was significantly lower than that of the half hip group(35.71%),and the difference was statistically significant(P<0.05).Conclusion Although the operation time and intraoperative bleeding are higher than those of hemiarthroplasty,total hip arthroplasty has fewer postoperative complications,is safer,and can restore hip function as soon as possible.
