利多卡因宫颈麻醉用于人工流产200例分析

Analysis of 200 Cases of Induced Abortion with Lidocaine Cervical Anesthesia

目的研究并分析利多卡因宫颈麻醉用于人工流产的临床效果。方法将2019年1月—2020年12月在泰安市第一人民医院门诊接受人工流产术的200例患者作为研究对象,以随机数表法分为利多卡因组与对照组,各100例。两组接受人工流产术患者均选择膀胱截石位,针对对照组不应用镇痛措施,对于利多卡因组,抽取5 mL2%的利多卡因进行宫颈麻醉。对比两组疼痛情况及宫颈松弛度。结果在宫颈松弛度上,利多卡因组宫颈松弛情况(100.00%)优于对照组宫颈松弛情况(24.00%),差异有统计学意义(χ^(2)=122.581,P<0.05)。利多卡因组的人工流产综合征(行动过缓、血压降低、心律失常、面色苍白、出汗、头晕、胸闷)发生率(8.00%)低于对照组,差异有统计学意义(χ^(2)=8.589,P=0.003)。结论对行人工流产术的患者采用宫颈注射利多卡因的方式,可以起到理想的麻醉效果,镇痛效果、宫颈松弛度均十分理想,可以减少患者的应激反应。

Objective To study and analyze the clinical effect of lidocaine cervical anesthesia in induced abortion.Methods 200 patients who underwent induced abortion in the outpatient department of Tai'an First People's Hospital from January 2019 to December 2020 were taken as the research object,and were divided into a lidocaine group and a control group of 100 patients by the random number table method.Patients undergoing abortion in both groups were selected for bladder truncation,and for the control group,no analgesic measures were applied.For the lidocaine group,5 mL of 2% lidocaine was drawn for cervical anesthesia,and the two groups were compared for pain as well as cervical relaxation.Results In terms of cervical relaxation,the lidocaine group had better cervical relaxation(100.00%)than the control group(24.00%),the difference was statistically significant(χ^(2)=122.581,P<0.05).The incidence of abortion syndrome(bradycardia,decreased blood pressure,arrhythmia,pallor,sweating,dizziness,chest tightness)in the lidocaine group was(8.00%)lower than in the control group,the difference was statistically significant(χ^(2)=8.589,P=0.003).Conclusion For patients undergoing abortion,the use of cervical injection of lidocaine can provide ideal anesthesia,with ideal analgesia and cervical relaxation,and can reduce the patient's stress reaction.
