畲药活血消积方对H型高血压痰瘀互结证患者脉搏波传导速度和踝臂指数的影响

Effect of She medicine Huoxue Xiaoji Fang on pulse wave velocity and ankle brachial index in patients with H-type hypertension with pattern of intermingled phlegm and stasis

目的观察畲药活血消积方对H型高血压痰瘀互结证患者脉搏波传导速度和踝臂指数(ankle brachial index,ABI)的影响。方法选取2020年9月至2022年1月于丽水市中医院就诊的64例H型高血压患者为研究对象,根据随机数字表法将其分为对照组和治疗组,每组各32例。对照组患者给予常规西药治疗,治疗组患者给予畲药活血消积方+西药治疗,疗程均为4周。比较两组患者的中医证候疗效、24h动态血压、同型半胱氨酸(homocysteine,Hcy)水平、肱踝脉搏波传导速度(brachial-ankle pulse wave velocity,baPWV)及ABI的差异。结果治疗组患者的总有效率显著高于对照组(P<0.05)。治疗后,两组患者的中医证候积分、24h收缩压(systolic blood pressure,SBP)、24h舒张压(diastolic blood pressure,DBP)、Hcy、baPWV均显著低于本组治疗前(P<0.05),ABI均显著高于本组治疗前(P<0.05);且治疗组患者的中医证候积分、24h SBP、24h DBP、Hcy、baPWV均显著低于对照组(P<0.05),ABI显著高于对照组(P<0.05)。结论畲药活血消积方联合西药治疗H型高血压痰瘀互结患者,能有效改善临床症状,降低血压和Hcy水平,改善血管弹性,值得临床应用。

Objective To observe the effect of She medicine Huoxue Xiaoji Fang on pulse wave velocity and ankle brachial index(ABI)in patients with H-type hypertension with pattern of intermingled phlegm and stasis.Methods A total of 64 patients with H-type hypertension treated in Lishui Hospital of Traditional Chinese Medicine from Sep-tember 2020 to January 2022 were selected as the research subjects.They were divided into control group and treat-ment group according to random number table method,with 32 cases in each group.The patients in control group were given conventional western medicine treatment,and the patients in treatment group were given the She medicine Huoxue Xiaoji Fang+western medicine treatment,and the course of treatment was 4 weeks.The difference on traditional Chinese medicine syndrome efficacy,24-hour ambulatory blood pressure,homocysteine(Hcy)level,brachial-ankle pulse wave velocity(baPWV)and ABI were compared between the two groups.Results The total effective rate of patients in treatment group was significantly higher than that in control group(P<0.05).After treatment,traditional Chinese medicine syndrome scores,24h systolic blood pressure(SBP),24h diastolic blood pressure(DBP),Hcy and baPWV in two groups were significantly lower than those before treatment(P<0.05),ABI were significantly higher than those before treatment(P<0.05).Traditional Chinese medicine syndrome score,24h SBP,24h DBP,Hcy,baPWV in treatment group were significantly lower than those in control group(P<0.05),ABI was significantly higher than that in control group(P<0.05).Conclusion She medicine Huoxue Xiaoji Fang combined with western medicine in the treatment of H-type hypertension patients with pattern of intermingled phlegm and stasis,can effectively im-prove the clinical symptoms,reduce blood pressure and Hcy level,improve vascular elasticity,worthy of clinical application.
