案例基础上超前护理对慢性乙型肝炎肝衰竭患者的影响

The influence of advanced nursing on patients with liver failure chronic hepatitis B based on case study

目的探讨案例基础上超前护理对慢性乙型肝炎肝衰竭患者恢复的影响。方法选取2019年2月至2021年5月浙江大学医学院附属第二医院临平院区收治的慢性乙型肝炎肝衰竭患者120例,根据随机数字表法将其分为对照组和试验组,每组各60例。对照组患者给予常规护理,试验组患者在对照组的基础上实施案例基础上超前护理,两组患者的护理周期均为患者入院至出院前,比较两组患者干预前后的焦虑自评量表(self rating anxiety scale,SAS)、抑郁自评量表(self rating depression scale,SDS)、健康调查量表36(36–item short form health survey,SF–36)评分及肝功能变化。结果干预后,两组患者的SAS、SDS评分及血清谷丙转氨酶(alanine aminotransferase,ALT)、总胆红素(total bilirrbin,TBil)水平均显著低于干预前(P<0.05),SF–36各维度评分和白蛋白(albumin,Alb)均显著高于本组干预前(P<0.05);试验组患者的SAS、SDS评分及血清ALT、TBil水平均显著低于对照组(P<0.05),SF–36各维度评分及Alb水平均显著高于对照组(P<0.05)。结论采取案例基础上超前护理,可缓解慢性乙型肝炎肝衰竭患者的负性情绪,提高患者对疾病的认知,提高生活质量,从而改善患者的肝功能。

Objective To explore the influence of advanced nursing on recovery of patients with chronic hepatitis B liver failure based on case.Methods A total of 120 patients with chronic hepatitis B liver failure admitted to Linping District,the Second Affiliated Hospital,Zhejiang University School of Medicine from February 2019 to May 2021 were selected and divided into control group and experimental group according to random number table method,with 60 cases in each group.Patients in control group were given routine nursing,and patients in experimental group were given advanced nursing based on cases on the basis of control group.The nursing cycle of patients in both groups was from admission to discharge.Self rating anxiety scale(SAS),self rating depression scale(SDS),36–item short form health survey(SF–36)scores and liver function changes were compared between two groups before and after intervention.Results After intervention,SAS and SDS scores,alanine aminotransferase(ALT)and total bilirrbin(TBil)levels in two groups were significantly lower than before intervention(P<0.05).SF–36 dimension score and albumin(Alb)were significantly higher than before intervention(P<0.05).SAS and SDS scores and serum ALT and TBil levels in experimental group were significantly lower than those in control group(P<0.05),SF–36 dimension scores and Alb levels were significantly higher than those in control group(P<0.05).Conclusion Advanced nursing on the basis of cases can relieve the negative emotions of patients with chronic hepatitis B liver failure,improve their cognition of disease,improve the quality of life,and the liver function of patients.
