小儿慢性鼻窦炎合并腺样体肥大临床应用腺样体切除的效果分析

Analysis of Clinical Effect of Adenoidectomy in Children with Chronic Sinusitis Complicated with Adenoid Hypertrophy

目的探究应用腺样体切除术治疗小儿慢性鼻窦炎合并腺样体肥大的疗效。方法选取2018年12月—2020年12月在深圳市龙岗中心医院住院治疗的60例慢性鼻窦炎合并腺样体肥大患儿为研究对象,依据治疗方式不同分为联合组(腺样体切除术+药物治疗)35例及对照组(单纯行药物治疗)25例。比较两组患儿治疗前、治疗3个月及治疗6个月后的慢性鼻窦炎评分及鼻内镜检查评分、总体治疗有效率。结果两组患儿治疗3个月后慢性鼻窦炎评分对比,差异无统计学意义(P>0.05),治疗6个月后联合组慢性鼻窦炎评分、鼻内镜检查评分分别为(0.93±0.31)分、(0.87±0.47)分均显著优于对照组,差异有统计学意义(t=5.212、6.233,P<0.05)。治疗3个月及治疗6个月后与治疗前相比,两组患儿的慢性鼻窦炎评分及鼻内镜检查评分均显著改善,差异有统计学意义(P<0.05)。联合组的治疗有效率为97.1%明显高于对照组88.0%,差异有统计学意义(χ^(2)=8.324,P<0.05)。结论腺样体切除治疗慢性鼻窦炎合并腺样体肥大患儿疗效显著,与单纯药物治疗相比有显著优势。

Objective To explore the efficacy of adenoidectomy in the treatment of children with chronic sinusitis complicated with adenoid hypertrophy.Methods A total of 60 children with chronic sinusitis and adenoid hypertrophy who were hospitalized in the Shenzhen Longgang Central Hospital from December 2018 to December 2020 were selected as the research objects.According to different treatment methods,they were divided into a combined group(adenoidectomy+drug treatment)with 35 cases and a control group(with drug treatment alone)with 25 cases.The scores of chronic rhinosinusitis,nasal endoscopy,and overall treatment efficacy were compared between the two groups before treatment,after 3 months of treatment,and after 6 months of treatment and perform statistical analysis.Results There was no significant differ⁃ence in the scores of chronic sinusitis between the two groups 3 months after treatment(P>0.05),the scores of chronic sinusitis and nasal en⁃doscopy in the combined group were respectively 6 months after treatment of(0.93±0.31)points and(0.87±0.47)points were significantly bet⁃ter than those of the control group,and the difference was statistically significant(t=5.212,6.233,P<0.05).In addition,after 3 months and 6 months of treatment,compared with before treatment,the scores of chronic sinusitis and nasal endoscopy in the two groups were significantly improved,and the difference was statistically significant(P<0.05).The effective rate of the combined group was 97.1%,which was signifi⁃cantly higher than that of the control group,88.0%,the difference was statistically significant(χ^(2)=8.324,P<0.05).Conclusion Adenoidectomy is effective in the treatment of children with chronic sinusitis and adenoid hypertrophy,and it has significant advantages compared with simple drug therapy.
