白虎加人参汤联合哌拉西林钠舒巴坦钠治疗老年重症肺炎患者的效果

Effects of White tiger decoction plus ginseng combined with Piperacillin sodium and Sulbactam sodium in treatment of elderly patients with severe pneumonia

目的:观察白虎加人参汤联合哌拉西林钠舒巴坦钠治疗老年重症肺炎患者的效果。方法:选取2018年9月至2020年11月该院收治的90例老年重症肺炎患者进行前瞻性研究,按随机数字表法分为研究组与对照组各45例。对照组采用哌拉西林钠舒巴坦钠治疗,研究组在对照组基础上联合白虎加人参汤治疗,比较两组临床疗效、症状消失时间、住院时间、治疗前后血清炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]水平、肺功能指标[第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)]水平,以及不良反应发生率。结果:研究组治疗总有效率为95.56%(43/45),高于对照组的80.00%(36/45),差异有统计学意义(P<0.05);研究组咳嗽消失时间、退热时间和住院时间均短于对照组,差异有统计学意义(P<0.05);治疗后,两组血清TNF-α、IL-6水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组FEV_(1)、FVC水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:白虎加人参汤联合哌拉西林钠舒巴坦钠治疗老年重症肺炎患者可提高治疗总有效率和肺功能指标水平,缩短症状消失时间和住院时间,以及降低血清炎性因子水平,效果优于单纯哌拉西林钠舒巴坦钠治疗。

Objective:To observe effects of White tiger decoction plus ginseng combined with Piperacillin sodium and Sulbactam sodium in treatment of elderly patients with severe pneumonia.Methods:90 elderly patients with severe pneumonia who were admitted to our hospital from September 2018 to November 2020 were selected for the prospective study and were divided into study group and control group according to the random number table method,45 cases in each.The control group was treated with Piperacillin sodium and Sulbactam sodium,while the study group was treated with White tiger decoction plus ginseng on the basis of those of the control group.The clinical efficacy,the symptom disappearance time,the hospitalization time,the serum inflammatory factor levels[serum tumor necrosis factor(TNF-α),interleukin-6(IL-6)]levels,the pulmonary function index levels[forced expiratory volume in one second(FEV_(1)),forced vital capacity(FVC)],and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the study group was 95.56%(43/45),which was higher than 80.00%(36/45)of the control group,and the difference was statistically significant(P<0.05).The cough disappearance time,the fever disappearance time and the hospitalization time in the study group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of serum TNF-αand IL-6 in the two groups were lower than those before the treatment,those of the study group were lower than those of the control group,and the differences were statistically significant(P<0.05).The levels of FEV_(1) and FVC in the two groups were higher than those before the treatment,those of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:White tiger decoction plus ginseng combined with Piperacillin sodium and Sulbactam sodium in the treatment of the elderly patients with severe pneumonia can improve the total effective rate of treatment and the pulmonary function index levels,shorten the symptom disappearance time and the hospitalization time,and reduce the serum inflammatory factor levels.Moreover,it is superior to single Piperacillin sodium and Sulbactam sodium.
