血清IL-12、Fib水平对新生儿坏死性小肠结肠炎的预测价值

摘要 目的分析血清白介素-12(IL-12)、纤维蛋白原(Fib)水平预测新生儿坏死性小肠结肠炎(NEC)的价值。方法选取2019年7月-2021年12月湖南中医药大学第一附属医院儿科收治的NEC患儿89例为NEC组,同时选取医院同龄健康新生儿85例为对照组。比较2组及不同分期NEC患儿血清IL-12、Fib水平的差异;Pearson分析评估NEC患儿血清IL-12、Fib水平与CRP、IL-6的相关性,ROC曲线评估血清IL-12、Fib水平对NEC的预测价值。结果NEC组新生儿血清IL-12、Fib、CRP及IL-6水平均显著高于对照组(t/P=35.143/<0.001、22.466/<0.001、21.876/<0.001、22.157/<0.001);不同NEC分期患儿血清IL-12、Fib水平比较,Ⅲ期>Ⅱ期>Ⅰ期(F/P=10.329/<0.001、40.267/<0.001)。Pearson分析显示,血清IL-12、Fib水平与IL-6、CRP均呈正相关(IL-12:r/P=0.714/<0.001、0.679/<0.001;Fib:r/P=0.706/<0.001、0.652/<0.001)。ROC曲线分析显示,血清IL-12、Fib及二者联合诊断新生儿NEC的AUC分别为0.754、0.700、0.842,二者联合检测的价值明显高于单项指标(Z/P=6.154/<0.001、5.024/<0.001)。结论新生儿NEC血清IL-12、Fib水平升高,二者联合检测对新生儿NEC具有良好的诊断效能,有利于临床早期提供针对性治疗。 Objective To analyze the value of serum interleukin-12(IL-12)and fibrinogen(Fib)levels in predicting neonatal necrotizing enterocolitis(NEC).Methods A total of 89 children with NEC admitted to the Pediatrics Department of the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from July 2019 to December 2021 were selected as the NEC group,and 85 neonates of the same age in the hospital were selected as the control group.The differences in serum IL-12 and Fib levels between the two groups and children with different stages of NEC were compared;Pearson analysis was used to evaluate the correlation of serum IL-12 and Fib levels with CRP and IL-6 in children with NEC.Predictive value of Fib levels for NEC.Results The serum levels of IL-12,Fib,CRP and IL-6 in the NEC group were higher than those in the control group(t/P=35.143/<0.001,22.466/<0.001,21.876/<0.001,22.157<0.001);different NEC Comparison of serum IL-12 and Fib levels in children by stage,stage III>stage II>stage I(F/P=10.329/<0.001,40.267/<0.001).Pearson analysis showed that serum IL-12 and Fib levels were positively correlated with IL-6 and CRP(IL-12:r/P=0.714/<0.001,0.679/<0.001;Fib:r/P=0.706/<0.001,0.652/<0.001).ROC curve analysis showed that the AUCs of serum IL-12,Fib and their combination in diagnosing neonatal NEC were 0.754,0.700,and 0.842,and the value of the combined detection of the two was significantly higher than that of the individual indicators(Z/P=6.154/<0.001,5.024/<0.001).Conclusion The serum levels of IL-12 and Fib in neonatal NEC are elevated,and the combined detection of the two has good diagnostic performance for neonatal NEC,which is conducive to providing targeted treatment in the early clinical stage.
