摘要
目的:从不同利益相关者角度出发,通过调查分析,为家庭医生签约制度的深入推进提供依据。方法:采用自行设计的问卷,对同一个区域的居民与家庭医生、基层医疗机构进行调查,描述与比较不同利益相关者的满意度。结果:不同利益相关者对家庭医生签约服务的评价呈现差异化,签约居民的整体满意度较高,重点人群满意度更高;家庭医生职业满意度较低,与学历呈负相关;基层医疗机构的满意度最低。结论:卫生行政部门应关注非重点人群健康需求,健全激励机制,优化基于绩效评价的资源配置;家庭医生应创新服务模式,落实签约服务内容,提高服务质量;基层医疗机构应加强区域内医联体联动效应,尤其是信息互联互通保障协作,加强资源的积聚和统筹,提升服务能力;引导居民转变就医观念,改进健康意识和健康行为,有效使用家庭医生签约服务。
Objective To provide evidence for promoting family doctor contracting services from the perspective of stakeholders.Methods A survey using a self-made questionnaire was conducted among residents,family doctors,primary care institutions in the same area to understand their distinct satisfaction towards family doctor contracting services.Results Different stakeholders showed different attitudes towards family doctor contracting services.The overall satisfaction signing residents was high,while the satisfaction of key groups was higher.Family doctors presented low satisfaction to their work,which was negatively associated with educational achievement.Conclusion Health administrative departments should pay attention to the health needs of non-key populations,improve incentive mechanisms,and optimize resource allocation based on performance evaluation.Family doctors should innovate service models,implement contracting service content,and improve service quality.Primary care institutions should strengthen the linkage of medical alliances in the region,especially guarantee cooperation based on information interconnection,strengthen the accumulation and overall planning of resources,and improve service capabilities.It is recommended to guide residents to change their concept of medical visit,improve their health awareness and behavior,and effectively use family doctor contracting services.
