自由体位联合应用分娩器具在初产妇分娩中的效果分析

Effect analysis of liberal position combined with delivery equipment among primiparas

目的分析初产妇实施自由体位联合应用分娩器具在改善分娩结局中的效果。方法随机抽取2020年6月—2021年5月于泰安市中心医院住院并采用自由体位联合分娩器具,如分娩球、瑜伽垫、分娩椅、挂墙扶手等待产的初产妇110例为自由体位组,随机抽取同期采用传统体位待产的初产妇115例为对照组。比较两组产妇产程时间、自然分娩率、产后出血量、新生儿Apgar评分、参考视觉模拟评分法(visual analogue score,VAS)所作的疼痛评估、采用焦虑自评量表(self-rating anxiety scale,SAS)得出的焦虑评分以及产后满意度等的差异。结果自由体位组产妇转剖宫产率(1.67%)明显低于对照组(9.57%),自然分娩率(98.33%)明显高于对照组(87.83%),总产程时间[(9.10±0.30)h]明显短于对照组[(13.30±0.36)h];产后出血量[(220±38)mL]低于对照组[(300±35)mL],VAS和SAS评分[(5.02±2.26)分,(31.54±3.30)分]均低于对照组[(7.78±1.64)分,(47.12±4.50)分],产后满意度调查表中选择满意与非常满意的人数比例(70.9%)高于对照组(24.3%),差异均有统计学意义(均P<0.05)。但新生儿Apgar评分两组间比较,差异无统计学意义(P>0.05)。结论产妇采用自由体位联合应用分娩器具分娩,可以有效提高自然分娩率,减少产后出血量,缩短总产程,减轻疼痛和心理焦虑状态,提高产后满意度。

Objective To analyze the effect of liberal position combined with the use of delivery equipment on the improvement of labour outcome.Methods Totally 110 primiparas who delivered with liberal position combined with delivery equipment including delivery ball,yoga pad,labour chair,and wall handrail in Tai’an City Central Hospital were recruited randomly as liberal position group,while 115 primiparas who delivered with traditional position in the same period were recruited randomly as the control group.The differences of labour time,natural childbirth rate,the amount of postpartum hemorrhage,neonatal Apgar score,visual analogue score(VAS),self-rating anxiety scale(SAS)score,and patients’satisfaction between the two groups were compared.Results Compared with control group,cesarean section converted rate in the liberal position group was lower(1.67%vs.9.57%),natural child birthrate was higher(98.33%vs.87.83%),the total labor time was shorter[(9.10±0.30)h vs.(13.30±0.36)h],the amount of postpartum hemorrhage was less[(220±38)mL vs.(300±35)mL],VAS was lower[(5.02±2.26)vs.(7.78±1.64)],SAS score was lower[(31.54±3.30)vs.(47.12±4.50)],and postpartum satisfaction rate was higher(70.9%vs.24.3%),and the difference was statistically significant(all P<0.05).Neonatal Apgar scores were not significantly different between the two groups(P>0.05).Conclusion Liberal position delivery combined with delivery equipment can effectively increase natural childbirth rate,reduce the amount of postpartum hemorrhage,shorten labour time,reduce pain and psychological anxiety,and improve postpartum satisfaction.
